Endoscopic carpal tunnel release: is it innocent?

Abstract

Objectives: We evaluated the results and complications of patients who underwent endoscopic release for carpal tunnel syndrome.
Methods: Endoscopic carpal tunnel release was performed in 30 hands of 28 patients with carpal tunnel syndrome using the Chow’s two-portal extrabursal technique. There were 23 females, and five males, with ages ranging from 26 to 60 years (mean age 40 years). All patients had persistent complaints for more than three months despite medical and/or physical therapy they had been receiving. Postoperative control examinations were performed at the 1st, 3rd, 6th, and 12th months. The mean follow-up was 17 months (range 6-28 months).
Results: On final follow-up examinations, limitations in activity resolved in 24 patients (80%), whereas two patients (7%) had minimal, three patients (10%) had moderate, and one patient (3%) had severe limitations. In one patient, total median nerve transection occurred during incision and primary neurorrhaphy was performed. Three cases developed contusions of the fourth and/or fifth digital nerves.
Conclusion: Surgical technique for carpal tunnel syndrome should be individualized for each patient and the surgeon. Although endoscopic carpal tunnel release seems to be an easy technique, it bears considerable potential risks for neurovascular injuries, making open surgery preferable for both certain patients and surgeons.

Endoskopik karpal tünel gevşetme: Masum bir girişim mi?

Abstract

Amaç: Karpal tünel sendromu nedeniyle endoskopik gevşetme uyguladığımız olgularda retrospektif sonuçlar ve komplikasyonlar değerlendirildi.
Çalışma planı: Karpal tünel sendromu saptanan 28 hastanın 30 el bileğine Chow'un tanımladığı çift portal ekstrabursal tekniğiyle endoskopik karpal tünel gevşetme uygulandı. Olguların 23'ü kadın, beşi erkekli (ort. Yaş 40; dağılım 24-60). Daha önce karpal tünel sendromu tanısıyla medikal tedavi ve/veya fizik tedavi uygulanmış, ancak yakınmaları üç aydan fazla devam etmiş, cerrahi tedavi endikasyonu olan olgular çalişmaya dahil edildi. Rutin kontroller ameliyat sonrasi l, 3, 6 ve 12. aylarda yapıldı.
Ortalama takip süresi 17 ay (dağılım 6-28 ay) bulundu.
Sonuçlar: Son kontrollerde olgulann 24'ünde (%80) aktivite kısıtlaması yokken, ikisinde (%7) minimal, üçünde orta (%10), birinde (%3) ileri derecede aktivite kısıtlaması gözlendi. Önemli komplikasyon olarak, bir olguda (%3) ameliyat sırasında total median sinir kesişi meydana geldiq bunun üzerine aynı seansta primer perifasiküler nörorafi uygulandı. Üç olguda (%10) dördüncü ve/veya beşinci digital sinir lezyonu gelişti.
Çıkarımlar: Karpal tünel sendromunda hangi tekniğin kullanılacağı her hasta ve cerraha göre özelleşmelidir. Endoskopik karpal tünel gevşetme, her ne kadar teknik olarak kolay görünse de, özellikle potansiyel nörovasküler yaralanma olasılığı da göz önüne alındığında açık cerrahi girişim tercih edilebilmelidir.
