Evaluation of functional results following surgical treatment of supracondylar femoral fractures

Abstract

Objectives: We evaluated functional results of patients who underwent surgical treatment for supracondylar femoral fractures.
Methods: Functional results of surgical treatment for supracondylar femoral fractures were evaluated in 23 patients (14 males, 9 females; mean age 34 years; range 14 to 64 years) with the use of the Neer and HSS (The Hospital for Special Surgery) scoring systems. The strength of the quadriceps muscle was measured using Cybex isokinetic testing. The mean follow-up was 52 months (range 13 to 160 months).
Results: The average HSS score was 93.8 (range 68 to 100) and the average Neer score was 90.1 (range 55 to 100). The average losses of extension strength at 60°/s and 180°/s were 27.30% and 22.13%, respectively. Statistically significant correlations were found between the losses of extension strength and the Neer and HSS scores (p<0.01). The results showed that the patient’s age and sex, the length of time between trauma occurrence and surgery, immobilization period, and the presence of subjective pain complaints had no significant influence on the HSS and Neer scores and on Cybex isokinetic test results (p>0.05).
Conclusion: Evaluation of the quadriceps muscle strength with the use of objective tests proved helpful in the determination of functional results of surgery following supracondylar femoral fractures.

Cerrahi olarak tedavi edilen suprakondiler femur kırıklı hastalarda fonksiyonel sonuçların değerlendirilmesi

Abstract

Amaç: Suprakondiler femur kırığı nedeniyle cerrahi tedavi uygulanan hastalarda elde edilen fonksiyonel sonuçlar değerlendirildi.
Çalışma planı: Suprakondiler femur kırığı nedeniyle cerrahi tedavi uygulanan 23 hasta (14 erkek, 9 kadın; ort. yaş 34; dağılım 14-64) çalışmaya alındı. Hastalar Neer ve HSS (The Hospital for Special Surgery) diz skorlama sistemlerine göre değerlendirildi. Kuadriseps kas gücünün objektif olarak belirlenmesi için Cybex izokinetik testi uygulandı. Hastalar ortalama 52 ay (dağılım 13-160 ay) süreyle izlendi.
Sonuçlar: Hospital for Special Surgery skoru 68-100 arasında (ort. 93.8), Neer derecelendirme skoru 55-100 arasında (ort. 90.1) bulundu. Açısal hızlardaki ekstansör güç kayıpları ortalamaları 60°/sn’de %27.30, 180°/sn’de %22.13 bulundu. Skorlama sistemi puanları ile 60°/sn ve 180°/sn açısal hızlardaki ekstansör güç kayıpları arasında istatistiksel olarak anlamlı korelasyon vardı (p<0.01). Yaş, cinsiyet, hastanın travmadan sonra erken veya geç ameliyat edilmesi, ameliyat sonrası immobilizasyon süresi, subjektif olarak ağrı şikayetinin olması gibi kriterlerin HSS ve Neer skorları ve Cybex izokinetik test sonuçlarına belirgin etkisi olmadığı belirlendi (p>0.05).
Çıkarımlar: Diz eklemi biyomekaniğinin en önemli unsuru olan kuadriseps kasının gücünün objektif yöntemlerle belirlenmesi, suprakondiler femur kırıklarının fonksiyonel sonuçlarının değerlendirilmesinde önemli bir yer tutmaktadır.
