Loading on the femoral component by the instron instrument in total hip endoprostheses and the examination of femoral sections under metollographic microscope

Abstract

In recent years, it has been observed that total hip endoprostheses have found an extensive field of application in the scientific branch of orthopaedics and traumatologly and that the results obtained are extremely reward promising. Yet, there seems to be certain complications arising during the late period, among which are the loosening of the acetabular and femoral components that give rise to problems. In an attempt to elucidate this problem, a study has been made in vitro on the bones obtained from the cadavers with the intention of invastigating the distribution aspects of loading prior to and after the insertion of prostheses in different position. The investigation has been effected by detecting the curve between load and crashing amount through applying bad by instron loading instrument and by examining under metallographic microscope the microstructural differeness arising after obtaining different sections at various levels. It has been concluded that the insertion of the prosthesis in the valgus position will eliminate the possibility of loosening provided that cement grasps the femoral stem of the prosthesis in every direction and the stem of the femoral component stays medially.

Abstract

Son yıllarda total kalça endoprotezleri, ortopedi ve travmatoloji bilim dalında yaygın olarak kullanılmakla ve erken sonuçların yüz güldürücü olduğu izlenmektedir. Ancak, geç dönemde ortaya çıkan bazı komplikasyonlar ki, bunların başında, asetabuler ve femoral komponentlerin gevşemesi sorunlar doğurmaktadır. Bu soruna bir açıklık getirmek amacıyla, in vitro olarak, kadavralardan elde edilen kemiklerde, protez yerleştirildikten sonra yük dağılımının özelliklerini araştırmak yönünden bir çalışma yapılmıştır. Araştırma, Instron yükleme cihazı ile yük uygulayarak, yük-ezilme miktarının eğrisini saptama ve femurlardan çeşitli seviyelerde kesitler alınarak ortaya çıkan mikrostrüktür farklılıkların metallografik mikroskopta inceleme yoluyla yapılmıştır. Sonuç olarak, protezin valgusta yerleştirilmesinin, sementin femoral komponent sapının medialde olması koşullarının yerine getirilmesi ile gevşeme olasılığının ortadan kalkabileceği fikrine varılmıştır.
