Ters Hill-Sachs lezyonu tedavisinde otogreft rekonstrüksiyonu

Abstract

Ters Hill-Sachs lezyonu posterior omuz çıkığında humerus başının anterior yüzünde çökme sonucu oluşan kırık sonrası görülen bir defekttir. Çalışmamızda, elektrik çarpmasına bağlı düşme sonucu sağ omuzda ağrı ve hareket kısıtlılığı nedeniyle, travma sonrası ikinci gün polikliniğimize başvuran 34 yaşında bir erkek hasta bildiriyoruz. Omuz radyografileri ve BT incelemelerinde sağ omuzda ters Hill-Sachs lezyonu olduğu görüldü. Travma sonrası beşinci gün ameliyat edilen hastanın, humerus başının açık redüksiyonu, deprese osteokondral yüzeyinin elevasyonu ve otogreft ile rekonstrüksiyonu yapılarak, glenohumeral eklem kapsülünün posterior yüzü onarıldı. Ameliyat sonrası 5 yıl takip edilen olgu Constant skoru ile değerlendirildi ve mükemmel sonuç elde edildi.

Abstract

Reverse Hill-Sachs lesion is a defect caused by the anterior compression fracture of the humeral head in posterior shoulder dislocation. We present a 34-year-old male patient with pain and limited joint mobility in the right shoulder due to a fall following electric shock 2 days prior to admission. Radiography and CT of the shoulder revealed posterior dislocation of the shoulder with a reverse Hill-Sachs lesion. The patient underwent open reduction of the humeral head, elevation of the depressed osteochondral surface, reconstruction using autograft, and repair of the posterior surface of the glenohumeral joint capsule 5 days following trauma. At the 5th year follow-up, the patient had a perfect outcome when evaluated using the Constant score.
