Augmentation of cement by K wires in filling the defect of bone

Abstract

The defect produced by malignant lesions in the long bones is treated by cement augmented internal fixation, but the defect produced by curettage of benign lesions is usually filled with cement only. Cement is mechanically weaker than cortical bone. Two models have been prepared to investigate the mechanical effect of two K wires embedded within the cement. The first model consisted of defects produced in the femurs of bovine filled with K wire and cement or with cement only. Taking into account the probability of anatomic variations of the bones, another model with two sets of cement blocks was prepared. In one of the sets, K wires were introduced into the cement block. Load for failure and the energy absorbed during failure were determined by three point bending test. The groups with K wires were more resistant in both models. It is concluded that addition of K wires is more convenient biomechanically in cases cementation applied.

Abstract

Uzun kemiklerin malign lezyonlarından ortaya çıkan defekt, sement ile kuvvetlendirilmiş internal tespit materyalleri ile giderilirken, benign lezyonlarda küretaj sonrası oluşan defekt genellikle yalnızca sement ile doldurulmakladır. Sement, mekanik bakımından kortikal kemikten daha zayıf bir malzemedir. Sement içine yerleştirilen iki adet çapraz K telinin mekanik etkisini araştırmak için iki model oluşturulan, ilk modelde dana femurlarında meydana getirilen defekt bir tarafta K teli ve sement ile doldurulurken, diğer tarafta yalnızca se-ment kullanılmıştır. Kemiklerde anatomik varyasyonlar göz önüne alınarak ikinci bir modelde, sementten oluşturulmuş blokların yarısının içine K telleri yerleştirilmiştir. Tüm örnekler Instron Universal Test cihazında üç nokta eğme testine tabi tutularak yük-defleksiyon münasebeti ve absorbe edilen toplam enerji tespit edilmiştir. Her iki modelde de K teli ilave edilen grup daha üstün neticeler vermiştir. Klinik olarak yorumlan-dığında; sementasyon uygulanan vakalarda, çapraz K teli ilavesinin biyomekanik olarak daha uygun olacağı görüşü kuvvet kazanmıştır.
