Quality of Turkish Online Information on Antibiotic Use in Periodontal Treatment: A Web-Based Analysis

Abstract

Objective: To evaluate the reliability, quality and infodemiological profile of online information on antibiotic use in periodontal disease treatment in Turkiye. Materials and Methods: Six Turkish search terms related to periodontal antibiotics were queried in Google and Yandex in March 2026. After deduplication and eligibility scree ning, 116 webpages were analysed. Reliability was assessed using Journal of the American Medical Association (JAMA) benchmarks, information quality using DISCERN and Global Quality Scale (GQS), and clinical alignment using a periodontal antibiotic stewardship check list based on contemporary periodontal guidance. Readability was estimated with the Ates man index. Group comparisons were performed using Kruskal-Wallis and chi-square tests. Results: The median DISCERN score was 38.0 (IQR: 31.0-48.0), and 64.7% of webpages were classified as poor or very poor. The median JAMA score was 1.0 (IQR: 1.0-2.0), with att ribution present in 19.0% and currency in 28.4%. Professional/university websites had higher DISCERN scores than private clinic websites (52.0 vs 36.0; p<0.001) and greater guideline align ment (6.0 vs 3.0; p<0.001). Only 21.6% required clinical evaluation before antibiotics, whereas 34.5% implied that antibiotics could independently treat gingival or periodontal inflammation. Antimicrobial resistance was mentioned in 27.6% of webpages. Higher search rank was weakly associated with lower DISCERN scores (rho=-0.24; p=0.010). Conclusion: Turkish online information on periodontal antibiotic use was highly vi sible but clinically uneven. Most webpages lacked transparent sourcing, risk communication and stewardship-oriented messages. These findings may help Turkish dental regulatory bo dies and professional organizations develop standardized, search-visible public guidance on periodontal antibiotic use.

Periodontal Tedavide Antibiyotik Kullanımına İlişkin Türkçe Çevrimiçi Bilgilerin Kalitesi: Web Tabanlı Bir Analiz

Abstract

Amaç: Türkiye'de periodontal hastalık tedavisinde antibiyotik kullanımıyla ilgili çevrimiçi bilgilerin güvenilirliğini, kalitesini ve infodemiyolojik profilini değerlendirmek. Materyal ve Metot: Mart 2026'da Google ve Yandex arama motorlarında periodontal antibiyotiklerle ilgili altı Türkçe arama terimi sorgulandı. Tekrarlayan kayıtların elenmesi ve uygunluk değerlendirmesinin ardından 116 web sayfası analiz edildi. Güvenilirlik *Journal of the American Medical Association* (JAMA) kriterleri kullanılarak; bilgi kalitesi DISCERN ve Küresel Kalite Ölçeği (GQS) ile; klinik uyumluluk ise güncel periodontal kılavuzlara dayalı bir periodontal antibiyotik yönetim kontrol listesi ile değerlendirildi. Okunabilirlik, Atesman indeksi ile tahmin edildi. Grup karşılaştırmaları Kruskal-Wallis ve ki-kare testleri kullanılarak yapıldı. Bulgular: Medyan DISCERN puanı 38,0 (Çeyrekler Arası Aralık [IQR]: 31,0-48,0) idi ve web sayfalarının %64,7'si zayıf veya çok zayıf olarak sınıflandırıldı. Medyan JAMA puanı 1,0 (IQR: 1,0-2,0) olup, sayfaların %19,0'unda kaynak belirtilmiş, %28,4'ünde ise güncellik sağlanmıştı. Profesyonel/üniversite web siteleri, özel klinik web sitelerine kıyasla daha yüksek DISCERN puanlarına (52,0'a karşı 36,0; p<0,001) ve daha yüksek kılavuz uyumluluğuna (6,0'a karşı 3,0; p<0,001) sahipti. Sayfaların sadece %21,6'sı antibiyotik öncesi klinik değerlendirme gerekliliğine değinirken, %34,5'i antibiyotiklerin diş eti veya periodontal enflamasyonu tek başına tedavi edebileceği izlenimini veriyordu. Antimikrobiyal dirençten web sayfalarının %27,6'sında bahsedilmişti. Daha yüksek arama sıralaması, daha düşük DISCERN puanları ile zayıf bir ilişki gösterdi (rho=-0,24; p=0,010). Sonuç: Periodontal antibiyotik kullanımına ilişkin Türkçe çevrimiçi bilgiler yüksek görünürlüğe sahip olsa da klinik açıdan tutarsızdı. Web sayfalarının çoğunda şeffaf kaynak kullanımı, risk iletişimi ve antibiyotik yönetimi odaklı mesajlar eksikti. Bu bulgular, Türkiye'deki diş hekimliği düzenleyici kurumlarının ve meslek örgütlerinin, periodontal antibiyotik kullanımı konusunda standartlaştırılmış ve arama motorlarında görünürlüğü yüksek halka yönelik rehberlik içerikleri geliştirmesine yardımcı olabilir.
