Parotis Bezi Kitlelerinin Sistemik İnflamatuvar Parametrelerle İlişkisi

Abstract

Amaç: Son yıllarda yapılan çalışmalar kanser ve inflamatuvar yanıt arasında bir ilişki olduğunu göstermiştir. Bu çalışmada inflamatuvar parametrelerden nötrofil/lenfosit oranı, trombosit/ lenfosit oranı ve sistemik immün inflamasyon indeksinin parotise ait kitlelerin ayırıcı tanısında kullanılıp kullanılamayacağını incelemeyi amaçladık. Gereç ve Yöntem: Bu retrospektif çalışmamızda Dicle Üniversitesi Tıp Fakültesi Kulak Burun Boğaz Polikliniği’ne parotiste kitle ile başvurmuş ve opere edilmiş toplam 100 hasta ile 70 sağlıklı birey kontrol grubu olarak çalışmaya alınmıştır. Hastaların 18’inin malign patolojisi varken 82’sinin patolojisi benign idi. Malign hasta grubu, benign hasta grubu ve kontrol grubu hemogram tetkikinden oranlanarak elde edilen nötrofil/lenfosit oranı, trombosit/lenfosit oranı ve sistemik immün inflamatuvar indeks parametreleri açısından karşılaştırıldı. Bulgular: Sistemik immün inflamatuvar indeks ve nötrofil/lenfosit oranı yönünden üç grup arasında anlamlı fark varken (p=0.006, p=0.000) trombosit/lenfosit oranı açısından gruplar arasında anlamlı farklılık gözlenmedi (p=0.285). Gruplar sistemik immün inflamatuvar indeks ve nötrofil/lenfosit oranı açısından ikili olarak karşılaştırıldığında, kontrol grubu ile benign hasta grubu arasında (p<0.017) ve kontrol grubu ile malign hasta grubu arasında (p<0.017) anlamlı farklılık görüldü. Ancak benign hasta grubu ile malign hasta grubu arasında istatistiksel olarak anlamlı bir farklılık izlenmedi (p=0.858, p=0.177) Sonuç: Sistemik inflamatuvar parametreler düşük maliyetli ve tam kan sayımı ile elde edildiklerinden ulaşılmaları kolaydır. Nötrofil/lenfosit oranı ve sistemik immün inflamatuvar indeks parametreleri parotis kitlesi olan ve olmayan bireylerde anlamlı farklılık gösterirken, parotisin benign ve malign patolojilerini ayırt etmede bu parametreler açısından istatistiksel olarak anlamlı farklılık izlenmedi.

Relationship of Parotid Gland Masses with Systemic Inflammatory Parameters

Abstract

Objective: Studies in recent years have shown a relationship between cancer and inflammatory response. In this study, we aimed to investigate whether neutrophil/lymphocyte ratio, platelet/ lymphocyte ratio and systemic immune inflammation index can be used in the differential diagnosis of parotid masses. Materials and Methods: In this retrospective study, 100 patients who came to Dicle University Faculty of Medicine Otorhinolaryngology outpatient clinic with a parotid mass and were operated were determined as the study group. 70 healthy individuals were determined as the control group. While 18 of the patients had malignant pathology, 82 of them were benign. Malignant patient group, benign patient group and control group were compared in terms of neutrophil/lymphocyte ratio, platelet/ lymphocyte ratio and systemic immune inflammation index parameters obtained from the hemogram examination. Results: While there was a significant difference between the three groups in terms of systemic immune inflammation index and neutrophil/lymphocyte ratio (p=0.006, p=0.000), no significant difference was observed between the groups in terms of platelet/lymphocyte ratio (p=0.285). When the groups were compared in terms of systemic immune inflammation index and neutrophil/lymphocyte ratio in pairs, there was a significant difference between control group and benign patient group (p<0.017) and between control group and malignant patient group (p<0.017), but there was no statistically significant difference between benign patient group and malignant patient group (p= 0.855, p=0.187) Conclusion: Systemic inflammatory parameters are easy to access as they are obtained with low cost and complete blood count. While neutrophil/lymphocyte ratio and systemic immune inflammation index parameters differed significantly in individuals with and without parotid mass, there was no statistically significant difference in distinguishing between benign and malignant pathologies.
