Aseptik Femur Kaynamamalarının Tedavisinde Çivi Değişimi Tedavisi ve Sonuçları / Exchange Intrameduller Nailing Treatment and Results in Aseptic Femoral Nonunions

Abstract

Amac: Femur kırığı sonrası ilk tedavisi kanal içi çivi implantı ile yapılmış fakat buna rağmen
kaynamama gelişen hastalarda daha kalın çivi ile yapılan çivi değişimi tedavisinin radyolojik ve
fonksiyonel etkinliğini değerlendirmektir.
Gereç ve yöntem: Daha önce kanal içi çivi ile tedavi edilmiş, aseptik kaynamama gelişen
ve en az bir yıl takibi olan 22 (15 erkek, 7 kadın) hasta çalışmaya alındı. Ortalama yaş 48
(dağılım: 22-86) yıl idi. Tüm hastalara en az 1 mm daha fazla kalınlıkta olmak üzere proksimali
ve distali kilitli kanal içi oymalı çivi yapıldı. Takipler sırasında çekilen radyografiler üzerinden
üç kortekste kaynama varlığı değerlendirildi. Hastaların fonksiyonel değerlendirmesi açısından
son takiplerinde desteksiz yürüme zamanı sorgulandı ve VAS (Vizüel analog skala) skalası ile
ağrı varlığı değerlendirildi.
Bulgular: Hastaların ortalama takip suresi 29 (12-47) aydı. İlk ameliyat göz önüne alındığında
ortalama çivi değişim zamanı 16 (9-22) aydı. Çivi çapı değişim miktarı ortalama 2.8 (1-6) mm
idi. Tüm hastalar desteksiz yürüyebiliyordu, tam solid kaynama vardı ve ortalama radyolojik
kaynama suresi 7 ( 4-13) aydı. Ek cerrahi gerektiren komplikasyon gözlenmedi.
Sonuç: Çivi tedavisi sonrası aseptik femoral kaynamama gelişen hastalarda çivi değişimi tedavisi
yüksek kaynama oranı ve fonksiyonel iyileşme ile sonuçlanan etkin bir tedavi yöntemidir.
