Açıklanamayan İnfertil Çiftlerde Erkek Faktörü: Sperm DNA Fragmantasyonu Değerlendirmesi

Abstract

Açıklanamayan infertilite günümüzde tüm infertil çiftlerin % 30-40 ını oluşturmaktadır. Yapılan tüm infertilite tetkiklerinde sonuçların normal değerlendirilmesi sonucu daha ileri tetkikler yapılması gerekmektedir. Bu konuda en çok yapılan çalışma erkek partnerin sperm DNA bütünlüğünün değerlendirilmesidir. Sperm kalitesi gebelik oluşumu ve devamı için önemlidir. Sperm DNA bütünlüğünü değerlendirmek için TdT-dUTP nick-end-labelling (TUNEL) yöntemi, tek hücre jel elektroforezi (COMET analizi), sperm kromatin yapı analizi (SCSA), flow sitometrik analiz, akridin orange testi ve floresan in situ hibridizasyon (FISH) yöntemi sıklıkla kullanılmaktadır. Bazı çalışmalarda testlerden sadece biri kullanılırken bazılarında iki ya da daha fazlası kullanılmıştır. Yapılan çalışmalar açıklanamayan infertilite tanısı almış erkeklerde sperm DNA fragmantasyonu oranının yüksek olduğunu göstermiştir. Bu çiftlerde ya gebelik oluşmamakta ya da erken gebelik kayıpları görülmektedir.
