Berrak Hücreli Over Kanserlerinin Retrospektif Analizi; Tek Merkez Çalışması

Abstract

Amaç: Over kanserlerinin büyük kısmı epitelyal kökenlidir. Epitelyal over kanserlerinin özel bir alt grubu olan ve nadir görülen berrak hücreli tip over
kanserlerinin özellikleri, klinik yaklaşım ve sağkalımlarının incelenmesi amaçlanmıştır.

Gereç ve Yöntem: Ankara Üniversitesi Tıp Fakültesi, Tıbbi Onkoloji Anabilim Dalı’nda 01.01.2010-31.12.2020 tarihleri arasında histopatolojik olarak
berrak hücreli tip epitelyal over kanseri tanısı alan takipli hastaların kayıtları geriye dönük olarak incelendi. Hastaların yaşları, tanı anındaki evreleri,
cerrahi sonrası histopatolojik tanıları, aldıkları kemoterapi rejimleri, genel sağkalım (GSK) ve progresyonsuz sağkalım (PSK) süreleri incelendi.

Bulgular: Çalışmaya alınan 11 hastanın ortanca yaşı 53 (22-70) yıl olarak saptandı. Hastaların 4’ü cerrahi evrelemeye göre evre I’de tanı alırken
diğer 7 hasta evre 3c olarak saptandı. Hastaların tanı anında ortanca CA 125 düzeyi 86 IU/mL (10-3122) iken, 3 hasta normal CA 125 düzeylerine
sahipti. Üç hastada eşlik eden endometriozis saptandı. Sadece bir hasta neoadjuvan kemoterapi alırken, tüm hastalara debulking cerrahi uygulandı.
Hastaların hepsi adjuvan kemoterapi aldılar. Hastaların takip süreleri sonunda PSK ve GSK incelendiğinde; ortanca PSK adjuvan tedavi sonrası uzak
metastaz ile nüks olan 7 hasta için 9 ay ortanca GSK 37 ay olarak saptandı.

Sonuç: Erken evre berrak hücreli tip epitelyal over kanserleri diğer erken evre epitelyal over kanserleriyle benzer sağkalıma sahip olmalarına rağmen,
ileri evrelerde daha kötü sağkalım görülmektedir.

Retrospective Analysis of Clear Cell Ovarian Cancers; A Single-center Study

Abstract

Objectives: Most ovarian cancers are of epithelial origin. The purpose of this study was to evaluate clinical characteristics, clinical approach and
survival of the rare clear cell type epithelial ovarian cancers, which are a special subgroup of epithelial ovarian cancers

Materials and Methods: The records of the patients who were followed up with a histopathological diagnosis of clear cell type epithelial ovarian
cancer between 01.01.2010 and 31.12.2020 in the Department of Medical Oncology at Ankara University Faculty of Medicine were reviewed
retrospectively. The patients’ ages, stages at the time of diagnosis, histopathological diagnosis after surgery, chemotherapy regimens, overall survival
(OS) and progression-free survival (PFS) were assessed.

Results: The median age of 11 patients included in the study was 53 (22-70) years. While 4 of the patients were diagnosed at stage I according to
surgical staging, the other 7 patients were found to be stage 3c. At the time of diagnosis, the median CA125 level of the patients was 86 IU/mL
(10-3122), while 3 patients had normal CA125 levels. Three patients had concomitant endometriosis. While only one patient received neoadjuvant
chemotherapy, all patients underwent debulking surgery. All patients received adjuvant chemotherapy after surgery. When the progression-free
survival and overall survival of the patients at the end of the follow-up period were examined, the median progression-free survival (mPFS) was 9
months for 7 patients who relapsed with distant metastases after adjuvant therapy. The median overall survival (mOS) was 37 months.

Conclusion: Although early-stage clear cell epithelial ovarian cancers have similar survival to other early-stage epithelial cancers, worse survival is
observed in advanced stages.
