two cases of malignant hypertermia

Abstract

In this paper we report 2 cases of MH induced by general anaesthesic. In the first case anesthesia was induced with thiopental succinlycholine, N2O/02 and halothane. Just before the termination of anesthesia tachicardia, aritmia, hypertension, hypertermia develophed. Diagnosis was confirmed by the detection of severe metabolic and respiratory acidosis, elevation of CPK. In the second case after the induction with O2 and halothane succinylcholine was administered. Just after the administration, masseter muscle spasm and generalized rigidite developed, severe metabolic acidosis and elevation of CPK were observed. Theraph consisted of hyperventilation with 100 % O2 external body cooling, correction of metabolic acidosis with NaHCO3. Also renal functions were supported.

İKİ OLGU NEDENİYLE MALİGN HİPERTERMİ

Abstract

Bu makalede genel anestezi ile indüklenen iki MH olgusu sunuyorum ilk olguda anestezi tiyopental, süksinilkolin, N20/02 ve halotan ile sažlandl. Anestezinin sonlandlrllmasmdan hemen önce tasikardi, aritmiler, hipertansiyon ve hipertermi gelisti. Derin metabolik ve respiratuar asidoz ve CPK yüksekliäi ile tani desteklendi, Îkinci olguda ise 02 ve halotan ile indüksiyonun ardlndan süksinilkolin uygulandl Uygulamanm hemen ardmdan masseter kas spazrm ve yaygln rijidite gelisti. Derin metabolik asidoz ve CPK yüksekliži gözlendi. Tedavi % 100 02 ile hiperventilasyon, eksternal sogutma ve metabolik asidozun NaHC03 ile düzeltilmesi seklindeydi. Aynl zamanda renal fonksiyonlar desteklendi.
