Ultrasonographic Evaluation Ot Deep Cavernosal Arteries Following Intracavernosal Vasoactive Agent İnjection

Abstract

In recent years, many diagnostic methods in the differentiai diagnosis ol impoltence are used to show penile vascular integrity. With this aim, as a pratical, efficient and diagnostic method, papaverineinjection is used with widespread acception. However, ultrasonographic evaluation of penile structures has been found to give much detailed knowledge in this concept and physicians began to apply these two methods in comhined form. 44 out of 88 patients who were evaluated with the complaints of
impotence underweni ultrasonographic evaluation of deep cavernosal arteries after papaverine injection in order to observe penile vascular structure and to detect vascular dilatation rate. Following the other diggnostic tests, the patients whom an organic etiology was suspected, test dose of papaverine was injected and the diameters of deep cavernosal arteries were evaluated. 3 of 14 patients who did not havea neurological, hormonal or psychogenic etiology seemed to have vascular pathology after this detailed evaluation. After defining ihe exaci etiological factors, these patients were taken in spesific therapy program.

İntrakavernozal Vazoaktif Ajan Enjeksiyonunu Takiben Derin Kavernozal Arterlerin Ultrasonografik Değerlendirmesi

Abstract

Son yıllarda empotans ayırıcı tanısında uygulanan araştırma, metodlarının büyük bir bölümü penil vasküler yapının bütünlüğünü göstermeye yöneliktir. Bu amaçla papaverin testi günümüzde etkili, pratik ve teşhis koydurucu bir yöntem olarak başarı ile uygulanmaktadır. Ancak son yıllarda ultrasonografik incelemenin bu konuda daha net görüntüler sağladığının gösterilmesi sonucu, her iki tanı metodunu kombine olarak uygulama eğilimi ortaya çıkmıştır. Empotans tanısı ile izlenmekte olan 86 olgunun 14 üne kliniğimizde vasküler yapıyı net olarak görüntüleyebilmek ve papaverin enjeksiyonu sonrasında ortaya çıkan vasküler değişiklikleri saptayabilmek amacı ile diğer uygulamaları takiben, papaverin testi ile ultrasonografik inceleme kombine olarak uygulanmıştır. Ayırıcı tanı amacı ile bu olgulara muhtemel organik patolojiyi belirlemek için test dozu papaverin enjekte edilmiş ve takiben derin kavernozal arterler bilepapaverin enjekte edilmiş ve takiben derin kavernozal arterler bilateral
olarak incelenerek çap ölçümleri yapılmıştır. Uygulama sonrasında psikojenik, hormonal, nörolojik açıdan normal bulunan ve vasküler etiololi açısından araştırılan bu olguların 3 ünde vasküler patoloji saptanmıştır, Kesin etiolojik nedeni ortaya konan olgular spesifik tedavi programına alınmışlardır.
