Bronkofiberoskopun Tüberküloz Tanısındaki Değeri

Abstract

Tüberküloz basilinin (ARB) balgamda görülmesi ve kültürde üremesiyle akciğer tüberkülozu tanısı kesinleşir. Balgamda ARB saptanamayan olgularda veya balgam çıkaramayanlarda bronkoskopi seçilecek bir yöntemdir. Bronkoskopik işlemin tüberküloz tanısını koymadaki değerini belirlemek amacıyla bu çalışma planlandı. Bronkoskopi öncesi alman en az 3 balgamda basil saptanamayan ve balgam çıkaramayan akciğer tüberkülozu olası 60 olguya bronkotiberoskopi yanıldı, Tüm olguların 35'de (& 58.3) bronkoskopik materyelde ARB saptandı. 35 olgunun 22'de ( 62.8) bronş lavajı kültüründe ARB üredi. 30 olgudan transbronşial akciğer biyopsisi (TBAB) alındı; ve bunların 3'nün (X 10) kültüründe üreme oldu. Bunlardan birinin mikroskopik incelemesinde de ARB görülmüştü. Bronkoskopik inceleme sonucu olguların 4de hem Tbe. hem de Akc. Ca. tanısının birlikte olduğu saptandı. Sonuçta; balgamda teksifle ARB negatif olgularda Tbc. tanisını sağlamada ve birlikte bulunan diğer hastalıkları da tanımlamada bronkoskopik incelemenin etkin bir yöntem olduğu kanısına varıldı.

VALUE OF BRONCHOFIBEROSCOPE IN TUBERCULOSIS DIAGNOSIS

Abstract

A definitive diagnosis of tuberculosis can be established only by isolation of tubercle bacilli in smears and.cultures. The. next diagnostic step, if the sputum is negative or can not be obtained, is usualIy bronchofiberoscopy. This study was planned in order to: eee diagnostic value of bronchoscopic procedures. 60 patients who have not bacilli in their —— and are Hot producing sputum was taken at least3 times prlor to undertaking bronchoscopic studies with lung tuberculosis. Among those cases in 35 of them (e 58.3) bacilli was established in fiberoscopic materials. Of those 35 cases bacilli was isolated in culture of bronchiolar lavage of 22, of them (X& 62.8), Transbronchial lung biopsy (TBLB) was performed on 30 cases In 3 of them (& 10) bacılli was found in culture of their TBAB, and in one of them mycobacterial smear was positive (X 3.3). Concomitant Tbe. with lung Ca. was found in 4 of the bronchoscopic specimens. Finally, bronchoscopic procedures have been especialiy helptul in ihe diagnostic evaluations of patients with the other combined diseases and if found negative bacilli by microscopy additional specimens should be taken.
