Relationship Between Anesthetic Method Applied to Femur Fractures and Postoperative Confusion
Abstract
1987 - 88 yıllarında İbn-i Sina Hastanesinde femur kırığı nedeniyle ameliyata alman 64 yaşm üzerinde 76 hastada anestetik tekniğin ameliyat sonu konfüzyonu etkileyip etkilemediği araştırıldı. Çalışmada hastalar 2 gruba ayrıldı. Birinci gruba rejional anestezi (Spinal ve epidural), ikinci gruba halotan anestezisi uygulandı. OBS (Organik beyin sendromu) skalası kullanılarak konfüzyon tayin edildi ve hastalarm % 9,2 sinde ameliyat sonu konfüzyon saptandı. Ameliyat sonu konfüzyonun daha önce mevcut olan mental depresyon ve kullanılan ilaçlarla ilişkisi vardı. Uygulanan iki farklı anestetik teknik arasında konfüzyon olasılığı bakımından fark bulunamadı. Genel anestezi grubunda mortalite sıfırdı, buna karşılık spinal anestezi grubunda bir hasta 4. gün oluşan pulmoner emboli nedeniyle 10. günde, epidural anestezi uygulanan bir diğer hasta ameliyat sonu 4. gün gelişen dolaşım yetmezliği ile kaybedildi . (Mortalite % 5,6).Bizim çalışmamızda, ameliyat öncesi mevcut olan hastalıklar, özellikle mental depresyon hikayesi, hastanın kullandığı ilaçlar ameliyat sonu konfüzyon ve komplikasyon gelişmesinde anestezi tekniğinden daha önemli faktörlerdi.
