Akut Myokard Infarktusunde 2 Boyutlu Ekokardıyografı İle Saptanan Bolgesel Duvar Hareket Bozukluklarının Elektrokardıyografık Bulgular İle Karsılastırılması

Abstract

Koroner bakım ünitesinde yatan akut myokard infarktüslü 91 hastada 2 boyutlu ekokardiyografi ile bölgesel duvar hareket bozuklukları incelenmiştir. Anterior MI'lı hastaların hepsinde duvar hareket bozukluğu saptanmış ve bu grubun % 42 nde apikal anevrizma bulunmugtur. Karsit olarak inferior MI'h hastalarin % 20 nde ekokardiyografik inceleme normal bulunmustur. Non-Q MI'lı hastalarin da % 63 nde bolgesel duvar hareket bozukları saptanmigtir, Ekokardiyografik duvar hareket bozuklugu gdsteren segment lokalizasyonu ile EKG'deki infarkt lokalizasyonu arasinda yuksek bir uyumluluk oldugu görülmüştür (% 84.8). Hastalarin % 6.6 nda EKG deki MI lokalizasyonu disinda, uzak bélgede duvar hareket bozuklugu saptanmistir. MI yanında myokard iskemisi de duvar hareket bozuklugu olusturabileceginden, akut MI'lı hastalarda bu «uzak bölge» duvar hareket bozukluklar: saptanmasi koroner arter hastaliginin beklenenden daha yaygin oldugu anlamına gelebilir.

Comparison of Regional Wall Motion Abnormalities Detected by 2-Dimensional Echocardiography with Electrocardiographic Findings in Acute Myocardial Infarction

Abstract

Regional left ventricular contraction abnormalities was evaluated in 91 patients with acute myocardial infarction. In all patients with anterior myocardial infarction (MI) various degrees of regional wall motion abnormalities was present and in 42 percent of them apical aneurysm was detected. In contrast, in 20 percent of patients with
inferior MI echocardiographic examination was normal. 63 percent of patients with non-Q MI displayed regional wall motion abnormalities. The site of left ventricular wall motion abnormalities correlated well with electrocardiographic prediction of the site of acute MI. In 6.6 percent of patients, in 2-dimensional echocardiographic examination, remote regional wall motion abnormalities was present that was not predicted from the ECG. Since both myocardial ischemia and infarction results in wall motion abnormalities, in patients with acute MI, this distant regional abnormality could suggest the greater extent of coronary artery disease than is predicted from the ECG alone.
