Romatoid Artrit Ve Ankilozan Spondilitli Hastalarda Serum Ferritin Düzeylerinin Akut Faz Reaktanı Olarak Değerlendirilmesi

Abstract

Konnektif doku hastalıkları, bakteriyel ve viral enfeksiyonla, neoplastik hastalıklar, travmatik lezyonlar gibi inflamatuvar durumlarda demir metabolizmasında, serum demiri ve transferrin düzeylerinde azalma ve serum ferritini konsantrasyonunda artma şeklinde sapmalar bildirilmiştir. Konvansiyonel olarak kullanılmakta olan eritrosit sedimentasyon hızı (ESR) C-reaktif protein, alfa2 globulin ayrıca fibrinojen ve haptoglobinlere ek olarak ferritininde bir akut faz reaktanı olduğunu ileri süren yayınlar mevcuttur. Bu çalışma, kronik inflamatuvar romatizmal hastalıklardan romatoid artrit ve ankilozan spondilitte serum ferritinin bir akut faz reaktanı gibi yükselip yükselmediğini araştırmak amacıyla, Gazi Üniversitesi Tıp Fakültesi Fiziksel Tıp ve Rehabilitasyon Anabilim Dalına başvuran ve yatırılarak izlenen 34'ü RA, 12'si AS'li toplam 46 hastada ve kontrol grubunu oluşturan 20 sağlıklı gönüllüde yapılmıştır. Hastalar ve kontrol grubunda ESR, CRP ve alfa2 globulinleri ile birlikte serum ferritini düzeylerine de bakıldı. Sonuçta, serum ferritininin RA ve AS'li hastaların akut fazlarında ESR ile paralel yükseldiğini ve bir akut faz reaktanı olarak bu hastaların takibinde değerli bir laboratuvar bulgusu olduğunu saptadık.

Evaluation of Serum Fetritin Lelvels as a Acute Phase Reactans in Rheumatoid Artritis and Ankylosing Spondylitis

Abstract

Iİnflammatory conditions such as; connective tissue disorders, bacterial and viral infections, proliferative diseases and traumatic injuries lessens iron metabolism, the level of serum iron concenitration and transferrin saturation with an increase of the serum ferritin level. İn addition to conventional technigues such as erithrocyte sedimentation rate (ESR), C-Reactive-Protein (CRP), alpha; globulin, fihrinogen and haptoglobin levels, also ferritin has been accepted to be an acute phase reactant in some reports. The purpose of this study was to found out if serum ferritin levels rised in chronic inflammatory diseases such as rheumatcid arthritis and ankylosing pondylitis. This research was carried out in Gazi University, Physical Medicine and rehabilitation Department and 46 patients and 20 controls were evaluated for their ESR, CRP, alpha; globulin and ferritin levels. The patient group was consisted of 34 rheumatoid arthritis and 12 ankylosing spondilis patients. Finally, we concluded that serum ferritin levels can be accepted as an acute phase reactant because we documented a positive correlation between serum ferritin levelsand ESR.
