Çift Kontrast Radyolojik Yöntemlerle Erken Mide Kanserlerinin Tanı Kriterleri

Abstract

Tokyo Women's Medical College Hastanesi Gastroenteroloji Enstitüsünde yapılan bu çalışmada, 193 hastada endoskopik olarak tespit edilen 109 erken mide kanseri vakasında, çift kontrast yöntemle
elde edilen radyografilerde erken mide kanserinin tanı kriterleri tespit edildi. Ayrıca kanserin boyutları radyolojik, rezeksiyon materyelinde makroskopik ve mikroskopik olarak karşılaştırıldı. Arada istatistiki olarak anlamlı farklılık bulunamadı.

Dıagnosıs Crıterıa For Early Stage Gastrıc Cancers Usıng Double Contrast Radıologıcal Methods

Abstract

In this study, the diagnostic criteria of the double contrast radiologic examination of 109 early gastric cancer out of 103 cases were investigated. Regarding distribution of the types of early gastric cancer, the depressed group (types II, ID, the elevated group (types 1, İla) the flat group (type Ilb) were examined a5 77 , 21 Y and2 “e respectively. In the depressed group, irregularity at the margin of the ulcer (82/14 96), convergance of the mucosal folds (73.80 9), sudden interruption of mucosal folds (63.00 4), rough granular barium coating (51.00 6) irregularity of the stomach wall (44.00 &), rod-like thickening of the mucosal fold (10.71 ”) small granular barium coating (9.52 6) and central protrusion (9.52 76) were observed. In the elevated group, rough granular barium coating ol 65.21 irregularity of the stomach-wall of 26.08 76, small granular barium coating of 4.24 Ye, microspiculation of the stomach-wall of 4.34 © and central protrusion of 9.52 72 were observed. The radiologic diagnosis of the fiat group was very difficult and small granular barium coating and microspiculation at the stomachwall were found.
In respect of the diameter of the lesions, no statistical differences were found in radiology, macroscopic examination of resected material and histopatological diameters. In conclusion, the double contrast radiologic examination that İs : superior to the convantional radiologic examination, is very important to improve the prognosis of the stomach cancer and should be used very widely.
