AKUT PANKREATİTİS TEDAVİSİ

Abstract

Konu ile ilgili son literatiir bulgularina ve tecriibelerimize göre, akut pankreatitis tanısı kesin ise, baslangicta yapilacak tedavi medikaldir ve ancak bazı durumlarda cerrahi girigim gerekliligi genel olarak benimsenmistir. Bu makalede, akut pankreatitis'in tibbi tedavisi tartışıldı ve ilgili literatiir gozden gegirildi. Ancak akut pankreatitis tanisinda siiphe varsa, erken laparotomi yapilmahdir. Laparatomi sirasinda akut pankreatitis bulunursa, pnkreasa internal dekompresyon uygulanmamaIichr. Safra yollart hastalığı saptandığı zaman, dış safra kanalının drenajı ile birlikte veya drenaj yapılmaksızın kolesistektomi veya kolesistostomi yararlı olabilir. Ba21 araştırıcılar, akut hemorajik (fulminant) pankreatitis icin subtotal veya total pankreatektomiyi Snermektedirler. Ayrica, safra tast, psödokist veya diğer bazt durumlar igin gerekli görülen elektif cerrahi girisim, akut episoddan takriben 6-8 hafta gectikten sonra yapılmalıdır.

Treatment Of Acute Pancreatıtıs

Abstract

According the recent literature and our experience related to the subject, it is generally accepted that as long as the diagnosis-of acute pancreatitis is reasonably certain the initial treatment should be medical, with surgical iptervention reconsidered under certain circumstances. The medical management of acute pantreatitis is discussed and recent literature on the subjects is reviewed in this paper. However, early operation is indicated if the diagnosis of acute pancreatitis is in doubt. When acute pancreatitis is discovered during the operation, internal decompression for the pancreas should not be performed. Cholecystectomy or cholecystostomy with or without drainage of the common bile duct, may be of some help in those situtatiuns where biliary disease exists. Some authors have advocated subtotal or total pancreatectomy to control acute hemorragic (fulminant) pancreatitis. Moreover, elective operation for proven gallstones and pseudocysts and some other conditi- ons should usually carried out about six to eight weeks after the acute episode.
