Renal Transplantasyonda Vasküler Komplikasyonlar

Abstract

Iliak ve renal arter arasında, önemli derecede çap uygunsuzluğu olmadıkça, anastomoz, UÇ-UCa yapılmalıdır. Anastomoz sırasında «dönme» olmamasına dikkat edilmeli, uzun tutulan alıcı ve verici arterlerinin açılanmaya bağlı olarak, oklüzyona sebep olabileceği hatırlanmalıdır. Renal arterde tromboz, stenoz gibi komplikasyonlardan koyunmak için, arterlerin intimalarının zedelenmemesine dikkat edilmelidir, Kanülasyon işlemi intima yırtılmasına sebep olmamalıdır, Donör renal arteri böbrek pelvisine doğru çok fazla dişseke edilmemelidir. Endarterektomi yapmak gerekmişse, plağın adventisiyaya yapıştığın dan iyice emin olunmalıdır. Ven anastomozlarmda eksternal iliak venanm serbestlestirilmesi için, internal iliak venin baälamp kesilmesine gerek yoktur. Lenfosel komplikasyonundan korunmak için, kesilen tüm lenfatikler bažlanmall, mutlaka negatif basmçll drenaj uygulanmalldlr. Anastomozlar tamamlandlktan sonra, böbrežin normal sertližine ve rengine kavustugu gözlenmeli, böbrek pelvis içindeki pozisyonuna konduktan sonra, arterde darllk, açllanma düâündürecek bir thrill olup olmadlžl arastlrllmalldlr. Canll donórlerden alman böbreklerde hemen fonksiyon beklenir. ilk 24-48 saatte herhangi bir zamanda böbrek fonksiyon kaybmda, vasküler komplikasyonlar düsünülmeli ve Slcak iskemi zamanmln, 1.5 saatten çok olamayacažl hatlrlanarak eksplorasyondan kaçmllmarnalldlr. Transplantasyonun birinci aymdan baslayarak gelisen ve medikal tedaviye cevap vermeyen hipertansiyonun, kronik rejeksiyon dismda, arter stenozuna da, bažll olabilecegi hatlrlanmall ve anjiografi uygulanmalldlr.
