The repair of complex or previously failed vesico-vaginal fistulas remains a surgical challenge. In this case, a 23-year-old woman with a long-standing vesico-vaginal fistula—developed after obstructed labor and unresponsive to three prior surgical attempts—was successfully treated using a pedicled omental graft. Through an open abdominal approach, a segment of omentum was mobilized and tunneled to the pelvic cavity, supporting the reconstruction of the urethra and bladder wall. Postoperative recovery was uneventful, and the patient regained full urinary continence within 60 days. This case demonstrates that omental grafting can offer definitive resolution for complex urogenital fistulas resistant to conventional techniques.
