Köpeklerde Acetylcholnne Arrest'i İle Koroner Anjiografi

Abstract

Selektif sine - koroner anjiografi yöntemi pahalı ve zahmetli oluşu sebebiyle araştırma laboratuvarlarında rutin olarak kullanılamamaktadır. Bu ihtiyacı basit bir yolla gidermek için acetylcholine arrest'i ile koroner anjiografi yöntemi bazı noktalarda modifiye edildi. Modifiye edilen metod, temel amacımız olan koroner cerrahisindeki deneysel çalışmalarımızda bize önemli değerlendirme olanağı sağladı.
Bildirilen bu yöntemde köpeklerde sol torakotomi yapılarak A. Subclavia'dan kalın bir kateter konuldu ve ascending aorta'ya kadar indirildi. Acetylcholine arrest'i yapıldı ve kateterden opak madde verilerek filim çekildi. Bu esnada descending aorta kapatıldı. Çekilen tek film seyyar, basit bir röntgen cihazı ile temin edildi.
Direkt yaklaşım olarak yorumlanabilen bu yöntemle 109 köpekte 210 anjiografi yapıldı. Başarısız anjiografi ve komplikasyon 8 deneyde oldu; 2 deneyde hava embolisi, 3 deneyde kateter kayması, 3 deneyde fibrilasyon görüldü. Geriye kalan 202 anjiogram tatminkâr ve net nitelikte idi.
Anjiogramların değerlendirilmesinde 109 köpekte % 76 oranında sol dominansi saptandı.
Sonuçlarımıza göre, her deney laboratuvarında kolayca kullanılabilecek olan bu metodla, tek filim ile yeterli nitelikte koroner anjiogram elde edilebilir.

Coronary Angiography in Dogs Using Acetylcholine Arrest Technique

Abstract

Selective cine - coronary angiography in dogs is a difficult procedure and rather expensive one. Therefore it is not done routinely in research laboratories. Iv•Ve have used a modified technique by using acetylcholine. We modified the coronary angiography with acetylcholine arrest technique at some points. This method has been very useful during our experimental studies on coronary artery surgery.
A left thoracotomy was done and a perfusion catheter passed from left subclavian artery to the ascending aorta. Cardiac arrest was done with acetylcholine. Opaque materiel given throug the catheter and film were taken, after clamping the descending aorta. The X - Ray machine used is a small, mobile simple one.
Study was carried out on 109 dogs with 210 angiograms. Negative results and with complication resulted on 8 experiments; air embolism in 2, catheter displacement in 3, and ventricular fibrilation in 3 respectively. Remaining 202 angiograms were clear and satisfactory.
During this study left coronary was dominant in 76 % of 109 dogs.
Our data shows that this method can easüy be used in a research laboratories for coronary artery angiography obtaining good results with a single film.
