Renal Arter Darlığına Yol Açan Hastalıklar ve Fibromüsküler Hiperplazinin Sıklığı: 29 Renal Arteriografinin İncelenmesi

Abstract

Renal artery narrowing arises from diverse causes, with atherosclerosis being most common, yet fibromuscular hyperplasia and other non-atherosclerotic lesions also play a significant role. We report a 17-year-old female presenting with malignant hypertension and left lumbar pain whose renal arteriogram revealed classic “string-of-beads” stenosis. Surgical biopsy confirmed fibromuscular hyperplasia. To assess the prevalence and radiologic characteristics of different etiologies, we retrospectively reviewed 29 renal arteriograms performed for hypertensive evaluation. Atherosclerotic stenosis accounted for 7 cases (24.1%), predominantly in older patients with coexisting aortic plaques and in 3 of these the perfusion was markedly impaired. Non-atherosclerotic causes comprised 12 cases (41.4%): including 2 with beaded appearance, single ostial lesions, probable embolic stenosis, arterial hypoplasia, and multiple focal narrowings. The remaining 10 cases (34.5%) had parenchymal kidney disease without aortic plaques. Radiographic features alone did not reliably distinguish atherosclerotic from non-atherosclerotic lesions. Our findings underscore that fibromuscular hyperplasia and other non-atherosclerotic renal artery diseases are more frequent than traditionally believed and should be considered especially in younger and female hypertensive patients.
