Venous Pressure Changes in Chronic Peripheral Arterial Occlusive Disease

Abstract

Background Peripheral arterial occlusive disease (PAOD) is commonly diagnosed by clinical examination and confirmed by arteriography, especially in surgical candidates. However, postoperative arteriography is often declined by patients, and other functional tests require specialized equipment.

Objective To evaluate the utility of peripheral venous pressure (PVP) measurement—via the great saphenous vein—as a simple, safe, and widely applicable method for assessing perfusion changes before and after surgical treatment of chronic PAOD.

Methods We studied 50 male patients (aged 18–68) with confirmed chronic PAOD at three levels: aorto-iliac (n=17), femoro-popliteal (n=13), and infrapopliteal (n=20). Ten healthy volunteers served as controls. With patients supine, a 12-gauge needle was inserted into the distal great saphenous vein at the medial malleolus; pressure was recorded via a fluid-filled manometer. Measurements were taken preoperatively and on postoperative days 3–5.

Results Control PVP averaged 124 mmH₂O. In aorto-iliac occlusions, mean PVP decreased to 65 mmH₂O (≈48% of normal). Femoro-popliteal occlusions showed a mean PVP of 79 mmH₂O (≈64% of normal). Infrapopliteal occlusions produced only minimal PVP reduction. Postoperatively, direct surgical reconstruction increased PVP by 20% toward normal, while indirect procedures yielded a 10–15% rise. Patients with persistent symptoms exhibited no significant postoperative PVP improvement.

Conclusion PVP measurement by saphenous vein cannulation is a practical, non-invasive test that reflects severity and level of arterial obstruction and can monitor surgical outcomes without specialized equipment.

Kronik Seyirli Periferik Arter Hastalıklarında Venöz Basınç Değişiklikleri

Abstract

Giriş Periferik arter oklüzif hastalık (PAOD), klinik ve arteriografik incelemeyle tanınır. Postoperatif arteriografi hastalar tarafından sıklıkla reddedilmekte, diğer fonksiyonel testler ise özel alet gerektirmektedir.

Amaç Basit, güvenli ve her merkezde uygulanabilir bir yöntem olarak, distal vena saphena magnadan periferik venöz basınç (PVB) ölçümünün PAOD’nin pre- ve postoperatif gidişini değerlendirmedeki yararını araştırmak.

Gereç ve Yöntem Çalışmaya aorto-iliak (n=17), femoro-popliteal (n=13) ve infrapopliteal (n=20) seviyelerde PAOD’lu 50 erkek hasta (18–68 yaş) ile 10 sağlıklı gönüllü alındı. Hastalar supin pozisyonda yatırıldı, medial malleolüs önünden 12 numara iğneyle vena saphena magna kanüle edildi ve manometreyle PVB kaydedildi. Ölçümler opere öncesi ile 3–5. postoperative günlerde tekrarlandı.

Bulgular Kontrol grubunda ortalama PVB 124 mm su idi. Aorto-iliak oklüzyonda PVB 65 mm suya (%48’e) geriledi. Femoro-popliteal oklüzyonda ortalama 79 mm su (%64’e), infrapopliteal oklüzyonda ise belirgin düşüş saptanmadı. Direkt cerrahi uygulananlarda postoperatif PVB %20, indirekt uygulananlarda %10–15 oranında normale yaklaştı. Semptomları devam edenlerde postoperatif PVB iyileşmesi gözlenmedi.

Sonuç Vena saphena magnadan PVB ölçümü, arteriyel oklüzyonun şiddeti ve seviyesi hakkında bilgi verir, cerrahi sonuçları izler ve özel ekipman gerektirmeyen pratik bir test olarak önerilir.
