Normal ve Normal Dışı Doğumlarda Göbek Kordon Kanında Oksijen Satürasyonu
Abstract
Background Umbilical cord blood analysis offers insight into fetal acid–base status and oxygenation during labor. Microtechniques now permit PO2 and pH measurements from <0.5 mL samples.
Objective To correlate umbilical venous blood pH, PO2, and PCO2 with neonatal APGAR scores in normal and complicated deliveries.
Methods Thirty-two term neonates were stratified: (1) uncomplicated multiparous/primiparous (n = 13), (2) difficult but vigorous births (forceps/vacuum, n = 7), and (3) depressed or preterm/vacuum/cord accidents (n = 12). Cord venous blood was drawn pre-clamping via Morris–Beard technique into heparinized syringes and analyzed for pH, PO2, and PCO2. APGAR at 1.5 min was recorded.
Results Group 1 (uncomplicated) mean pH 7.153 (range 7.040–7.343), PO2 19 mm Hg (15–27), PCO2 58 mm Hg, HCO3– 23 mEq/L. Prematures in group 3 (n = 5) showed mean pH 7.150 and PO2 14.2 mm Hg. Depressed neonates had respiratory acidosis (pH 6.98–6.94; PCO2 72–95 mm Hg; PO2 10–5.5 mm Hg). APGAR and blood gas deviations were most pronounced in complicated/difficult deliveries.
Conclusions Umbilical venous pH and PO2 correlate with neonatal condition. Combined APGAR–blood gas evaluation enhances prognostic assessment, especially in complicated births.
