Servikal Kordon Travmalarında Ön Dekompresyon ve Füzyon (Cloward) Cerrahisinin Rolü

Abstract

This study reviews the role and surgical technique of anterior decompression and interbody fusion (Cloward procedure) in the treatment of cervical spinal cord injuries. Despite advancements, conventional conservative management and posterior approaches have not consistently demonstrated sufficient efficacy in such injuries. The anterior approach offers direct access to the intervertebral disc and allows decompression of the spinal cord without manipulation, which is particularly advantageous in cases involving compressive disc herniation or fracture-dislocations. Clinical outcomes from a series of cases are presented, emphasizing improved postoperative recovery, reduced neck pain, shorter hospitalization, and earlier initiation of rehabilitation, especially in non-quadriplegic patients. Although outcomes in complete quadriplegic patients remain limited, anterior fusion facilitates patient care and provides psychological benefits. Based on the findings and existing literature, the Cloward technique is suggested as a favorable surgical option for appropriately selected cases of cervical spine trauma.
