Hacettepe Üniversitesi Tıp Fakültesi Erişkin Kardio - vasküler Cerrahi ile Erişkin Kardioloji Bölümünde 1965 - 1970 seneleri arasında, takriben 5 yıllık süre içinde, 96 vak'ada mitral valvine açık kalp cerrahisi ile müdahele edilmiş ve takibe alınmıştır. 'Takip süresi en uzun 44 ay olup, ortalama 10 aydır. 96 vak'anın 25 inde kapak değiştirilmesine lüzum görülmemiş, sadece açık mitral valvulotomi yapılmıştır. 25 vak'ada operatif mortalite 9 8, hastane ve geç mortalite o 0 dır. Geri kalan 66 vak'ada suni kapak takılmış olup bunlarda operatif mortalite 90 20, hastane mortalitesi 96 20 ve
geç mortalite Go 0 dır. Gerikalan 66 vak'ada suni kapak takılmış olup, bunların 16 sında Starr - Edwards, 50 sinde Kay - Shiley kapağı kullanılmıştır. Kay - Shiley kapağı kullamlan 50 vak'ada total mortalite Go 18 olup, kapak değiştirilen 66 vak'ada operatif mortalite 5 0 dır.Kapak değiştirilen vak'alarda trombo - embolism nisbeti 6 13.6 ve SBE nisbeti ise 96 9 dur. Suni kapak takılan bu vak'alarda kapagın bir tek devamlı sütürle konduğu hastalarda hiç bir tombo - embolik hadise görülmemiştir. İster suni kapak kullanılsın, isterse sadece açık mitral kommissurotomi yapılmış olun, hastaların amelirat sonu istifadeleri iyi veya çok iyi olarak nitelerdirilmiştir.
Between 1965 and 1970, at the Departments of Adult Cardiovascular Surgery and Adult Cardiology, Hacettepe University Faculty of Medicine, 96 cases underwent open-heart surgery for mitral valve replacement and were followed up over a period of approximately five years. The longest follow-up period was 44 months, with an average of 10 months. Valve replacement was not deemed necessary in 25 of the 96 cases; only open mitral valvulotomy was performed. In 25 cases, operative mortality was 9.8, hospital mortality was 0.00, and late mortality was 0. In the remaining 66 cases, artificial valves were implanted, with operative mortality of 90.20, hospital mortality of 96.20, and late mortality of 0.00. In the remaining 66 cases, artificial valves were implanted; 16 of these involved the Starr-Edwards valve and 50 involved the Kay-Shiley valve. Total mortality in the 50 cases using the Kay-Shiley valve was 18, and operative mortality in the 66 cases requiring valve replacement was 50. The thromboembolism rate in the valve replacement cases was 6.136, and the SBE rate was 96.9. No thromboembolism or embolism was observed in these patients who received a single continuous suture. Whether the artificial valve was used or only open mitral commissurotomy was performed, the patients' postoperative outcome was considered good or excellent.
