Doğuştan Vajinal Atrezi Olan İki Olguda Wharton Yöntemi ve Vajinal Protez Uygulaması ile Yapay Vajina Oluşturulması

İki konjenital atresia vaginalis vakası sunulmuştur. Birinde tam vajinal eksiklik ve iç genital organ yokluğu, diğerinde parsiyel vajina oluşumu ve normal iç genital organlar mevcuttu. Her iki olguda, Wharton usulüyle perineal alanda mesane-üreter-anüs hattı arasına 12 cm × 3 cm boyutunda tünel açıldı ve östrojenli merhekle kayganlaştırılmış vaginal protez yerleştirildi. Postoperatif dönemde 5–7 gün mesane kateteri takılı kaldı; protez 6 ay boyunca gündüz-gece, sonraki 6 ay boyunca yalnızca gece kullanıldı; günlük serum fizyolojik lavaj ve protez temizliği uygulandı. Erken dönemde neovajina patente kaldı, komşu organ hasarı olmadı ve başlanan epitelleşme izleniyor. Wharton usulü, kolay uygulanabilirliği ve güvenliğiyle vajinal patens ve cinsel işlev kazandırmada etkili bir yöntemdir.

Vaginal atresia, whether congenital or acquired, deprives women of menstrual outflow and sexual function. We present two adolescents with congenital vaginal aplasia: one with complete absence of the vaginal canal and internal genital organs, and another with partial canal formation and intact uterus and tubes. Both underwent Wharton’s simple perineal technique: a 12 cm × 3 cm tunnel was dissected between urethra/bladder and rectum, and an estrogen-lubricated vaginal prosthesis was inserted and secured. Postoperatively, the catheter remained for 5–7 days, and the prosthesis was daily irrigated and used continuously for six months, then nightly for another six months. Early results show a patent, epithelialized neovagina without injury to adjacent organs. Wharton’s method offers a straightforward, reproducible approach to restore vaginal patency and preserve future fertility if internal organs are present.
