Abstract

E. coli menenjitleri erişkinlerde oldukça nadirdir. Kliniğimizde 25 yıl boyunca gözlenen üç gram negatif menenjit vakasının tümü lomber ponksiyon sonrası gelişmiştir. Bu çalışmada cerrahi girişim öyküsü olmayan, odaklanmış bir enfeksiyon belirlenemeyen ve primer E. coli menenjiti tablosu gösteren 62 yaşında bir kadın hasta sunulmaktadır. Baş ağrısı, ense sertliği, ajitasyon ve bilinç bozukluğu gibi semptomlar gözlenmiş; beyin-omurilik sıvısı ile boğaz kültürlerinden E. coli üremesi tespit edilmiştir. Klinik seyir, menenjokok salgınlarında gözlenen patojenezle benzerlik göstermekte olup, bakterinin farenks yerleşiminden hematojen yayılım sonucu leptomenenjlerde oturduğu düşünülmektedir. Literatür değerlendirmesi ve ilgili histopatolojik veriler tartışılmıştır.

Abstract

Escherichia coli meningitis is rarely encountered in adults. Over 25 years, only three cases of gram-negative meningitis were observed at the clinic, all following diagnostic lumbar punctures. This report presents a unique case of primary E. coli meningitis in a 62-year-old woman with no identifiable infection focus or prior surgical intervention. Symptoms included headache, neck stiffness, agitation, and altered consciousness. Cerebrospinal fluid cultures yielded E. coli, also isolated from throat cultures. The clinical progression suggests hematogenous spread from pharyngeal colonization, resembling the pathogenesis seen in meningococcal outbreaks. Literature on similar cases and pathological findings is reviewed.
