Artificial Pacing of the Heart Using Transvenously Applied Endocardial Electrodes: A Case Presentation and Method Discussion
Abstract
This clinical report discusses a case of idiopathic hypertrophic subaortic stenosis (IHSS) in an 18-year-old male who exhibited electrocardiographic findings mimicking anterior myocardial infarction. The patient presented with exertional headaches, dizziness, and occasional syncope. Clinical and hemodynamic studies, including apexcardiogram, phonocardiogram, vectorcardiogram, and left heart catheterization, revealed hallmark signs of IHSS—subvalvular gradient, systolic pressure drop across the outflow tract, and post-extrasystolic potentiation. Coronary angiography was normal, ruling out ischemic disease. The study emphasizes the diagnostic challenge in distinguishing IHSS from actual myocardial infarction based on ECG alone, especially in young individuals with normal coronaries.
