Abstract

Ankara Üniversitesi Tıp Fakültesi, Gastroenteroloji Kliniğinde 1966 - 1973 seneleri arasında görülen 16 akut pankreatitis'li vak'a takdim edildi, seyir ve prognoz açısından incelendi. Bu vak'alardan 10'u en az bir sene, 6'sı en az iki sene takip edilmişlerdir. Vak'aların takipleri sırasında görülen gelişmeler ayrıntıları ile belirtildi. Akut episod sırasında uygulanan tedavi yönteminin, prognoz açısından önemli olduğu bulundu. Tanı kesin ise, akut episod sırasında rutin tıbbi tedavi uygulanmalıdır. Özellikle tanıda güçlük gösteren ve akut abdomen ile başvuranlarda, laparotomi denenebilir. Ancak akut pankreatitis bulunduğunda, pankreasta internal dekompresyon esasına dayanan cerrahi yöntemlerin uygulanması sakıncalı bulunmuştur. Öte yandan, safra taşı veya diğer nedenlerle elektif operasyon, akut episod'dan 6-8 hafta geçtikten sonra yapılmalıdır.

Akut Pankreatitis Vak'alarımızda Seyir Ve Prognoz

Abstract

The Survival and Prognosis in the Acute Pancreatitis Sixteen acute pancreatitis were seen in the Gastroenterology Department of the Ankara University of the Medical School between 1966 up to 1973. These cases were presented and investigated by the survival and prognostic point of view. Ten cases with acute pancreatitis were followed at least for one year, the six of these at least for two years. The interesting meanifestations during the survival were documented in detail, The management during the acute episode were found as an important point for the prognosis. The routine treatment of the acute episode should be conservative es long as the diagnosis is reasonabiy certain. It would therefore seen rational to suggest that laparotomy is indicated when the diagnosis is guestionable in the patient with the acute abdomen. However, when acute pan» creatitis in discovered during laparotomy, internal decompression for the pancreas should not be performed. Mereover, electiveoperation for proven galistones or scme other conditions should usually carried out about six to eight weeks after the acute episode.
