Cerrrahi Pies Arteriografisi (Spesmen Arteriografisi) ve Tümör İle İltihabın Ayırıcı Tanısında Kullanılması

Abstract

Bizim, piyesler üzerinde yaptığımız incelemelerden elde ettiğimiz bulgularla, literatür karşılaştırılacak olursa, bazı benzerlik ve ayrılıkların ortaya çıktığı görülmektedir. Ayrıntılı bir literatür mukayesesine girmeden, elde ettiğimiz sonuçları şöyle özetleyebiliriz ;
1 — Pies arteriografilerinde barium sülfat yeterli bir opaklaştırıcı olarak kabul edilebilir.
2 — Pies arteriografileri ile, malign tümör ve iltihap. arasında ayırt teşhisde, canlıda yapılan anjiografiye oranla, daha kesin objektif bulgular elde edilmektedir.
3 — Bizim vak'alarımızın çoğunda, kapiller damarların itilmesi sonucu, tümör ile normal doku arasında bir opak hudut meydana, geldiği dikkati çekmektedir, Bu bulguya literatürde rastlamadık.
4 — Bazı proçeslerde, lezyon alanında, ampute damar görünmesi bu çalışmamızdan çıkan sonuca, göre, patognomonik değildir; iltihap, ülser ve adenokanserlerde görülmektedir,
5 — Bizim çalışmamızda hemen bütün malign tümörlerde saptadığımız ve bizce ayırt teşhis yönünden en önemli patogno- monik belirti: tümörü besleyen damarlardan bir tanesinin çapının başlangıçtan sonü kadar değişmeden aynı kalınlığı koruması'dır. Bu özelliğe literatürde hiç değinilmemiştir.

Surgical Pies Arteriography (Specember Arteriography) And Its Use In The Differential Diagnosis Of Tumor And Inflammation

Abstract

Comparing the findings from our studies on the specimens with the literature reveals some similarities and differences. Without delving into a detailed literature comparison, we can summarize our results as follows:

1 — Barium sulfate can be considered a sufficient opacifier in Pies arteriographies.
2 — Pies arteriographies provide more accurate objective findings than in vivo angiography in distinguishing between malignant tumors and inflammation.
3 — In most of our cases, it is noteworthy that an opaque border forms between the tumor and normal tissue as a result of capillary displacement. We have not encountered this finding in the literature.
4 — In some processes, the appearance of amputated vessels in the lesion area is not pathognomonic, according to the conclusion of this study. It is seen in inflammation, ulcers, and adenocarcinomas.
5 — In our study, the most important pathognomonic sign we found in almost all malignant tumors, and in our opinion, the most important diagnostic finding, is that the diameter of one of the vessels feeding the tumor maintains the same thickness from beginning to end. This feature has never been mentioned in the literature.
