Abstract

Doğuştan veya spastik serebral palsi ile ilişkili sorunlu kalça çıkıklarını azaltma tekniği açıklanmaktadır. Bu teknik, femoral klempleme ve açık redüksiyonu birleştiren basit bir tekniktir. Ameliyat öncesi traksiyon ve yumuşak doku gevşetme işlemine gerek yoktur, bu da redüksiyondan önce istenmeyen femur başı osteoporozunu önler. Vaka çalışmaları, diğer yöntemlere kıyasla avasküler nekroz ve diğer komplikasyonların daha düşük insidans gösterdiğini ortaya koymuştur. Bu tür komplikasyonlar meydana gelse bile, çıkık redüksiyon halinde kaldığı için gerekli rekonstrüktif prosedürlerin uygulanması daha kolaydır.

A New Method Of Treatment For Congenital Dislocation Of The Hip Joint In Older Children And First Analysis Of 1361 Cases

Abstract

The technique described is for reducing the diffucult dislo cated hip, either congenital or associated with spastic cerebral palsy. The operative technique is simple, combining the principles of open reruction with femoral shortening. Preoperative traclion and soft tissue release prior to reduction is not necessary and the consequent undesirable osteoporosis of the femoral head perior to reduction is avoided. The follow-up the these challenging cases reveals no avascular necrosis or other significant complication. While some form of degenerative joint disease or avascular necrosis may still supervene, the dislocations remain reduced and future reconstructive work with the hips, which may be necessary, should be easier and more rewarding than before.
