Abstract

A investigation of the role of preserving the ANTRUM - PYLORUS - DUODENUM mechanism in occourence of dumping syndrom
In the surgical treatment of peptic duodenal ulcer, most of the symptoms seen after vagotomy combined with pyloroplasty (epigastric discomfort - sweting - tachyoardie - disiness) have awoked the idea that drainage operations must probably on the results. In this article we have investigated the role of the preservation of the ANTRUM - DUODENUM mechanism in occurence of dumping syndrome. For this purpose, 89 cases treated with highly selective vagotomy without drainage and 15 cases of highly selective vagotomy with drainage, treated in the same period between 1971 - 1974 in our clinic, have been analysed retrospectively. Dumping cocurence in highly selective
ANTRUM Pylorus vagotomy without drainage group was found to be % O, while it was 33,5 % in the drainage group.
Our results and other reports seen in recent literature following H.S.V. without drainage or the preservation of the pylorus with distal' gastrectomy have showed the importance of the ANTRUM - PYLORUS - DUODENUM mechanism in the occurence of dumping syndrome.

Abstract

Duodenum ülseri için yapılan ameliyatlarda vagotomiye eklenmiş piloroplastilerden sonra tesbit edilen semptomların çoğunun (Epigastrik rahatslzllk - vazomotor bozukluk - halsizlik basdönmesi) drenaj ameliyatlarlrnn klinik neticelerinin sebep oldugu görüldünde çahsmalar sürdürülmektedir.Biz bu incelemede ANTRUM - DUODENUM birližinin korun-masml-n dumping olusumu üzerindeki degerini 1971 - 1974 Ylll•an arasmda klinižimizde uyguladlžlrmz DRENAJSIZ - Y.S.V.'li 89 ile sürede DRENAJLI - Y.S.V. ile tedavi edilen 15 hasta üzerinde klinik ve radyolojik olarak arastlrdlk. Bu 2gruptan DRENAJSIZ - Y.S.V. ile tedavi edilen grupta DUMPiNG olusumunun % O olmasl yanmda, DRENAJLI grupta bu oran % 33,5 dežerinde bulunmustur.Hem bizim bulgularlmlzm sonuçlarl hem de literatürde drenajslz - Y.S.V. ile çahsan arastlncžlarla, pi]oru koruyarak distal gastrektomi yapan gruplarm yaylnladlklarl raporlann sonuçlarl, ANTRUM - - DUODENUM birližinin bozulmasž. mn dumping olusumundaki sorumlulugunu ortaya çıkarmıştır.
