Abstract

This clinical study reviews the application of subtrochanteric compression osteotomy in 19 patients (21 procedures) treated between 1967–1972 for coxarthrosis at Ankara University. The procedure aimed to reduce pain, enhance mobility, and delay advanced interventions in cases with preserved hip motion. The technique involved medial displacement and iliopsoas tenotomy, using Mueller–Harris nails and compression plates. Key outcomes included union in all cases, reduced pain (82.3% in rest, 76.4% during loading), improved mobility in 53%, and radiological signs of regeneration in most patients. No thromboembolism, death, or pseudoarthrosis occurred. The authors emphasize mechanical reconfiguration to modify biological degeneration. In selected patients with early to moderate osteoarthrosis and adequate joint motion, subtrochanteric compression osteotomy is proposed as an effective, minimally invasive alternative.

Abstract

Bu klinik çalışmada, 1967–1972 yılları arasında A.Ü. Tıp Fakültesi Ortopedi ve Travmatoloji Kliniğinde koksartroz tanısıyla tedavi edilen 19 hastaya (21 osteotomi) uygulanan subtrokanterik kompresyonlu osteotomi tekniği sunulmuştur. Ağrıyı azaltmak, hareket kabiliyetini artırmak ve ileri müdahaleleri geciktirmek amacıyla mediyal deplasman ve iliopsoas tenotomisiyle birlikte Mueller–Harris çivi ve kompresyon plakları kullanılmıştır. Takiplerde tüm vakalarda kaynama sağlanmış, istirahatte ağrı %82.3, yüklemede ağrı %76.4 oranında azalmıştır. Mobilite %53 hastada artmış, çoğu hastada radyolojik rejenerasyon bulguları saptanmıştır. Tromboembolizm, ölüm veya psödoartroz gözlenmemiştir. Yazarlar, biyolojik dejenerasyonu değiştirmek için mekanik kuvvetlerin yeniden yapılandırılmasını vurgulamakta; erken ve orta evredeki kalça osteoartriti vakalarında bu cerrahî yöntemi etkili ve düşük riskli bir seçenek olarak önermektedir.
