İntratorasik Meningosel: Nadir Bir Vaka Üzerine Gözden Geçirme

This report presents a rare case of intrathoracic meningocele diagnosed at Ankara University Medical Faculty, Thoracic and Cardiac Surgery Department. Initially suspected as a neurogenic tumor due to left shoulder pain and radiographic findings, surgical exploration revealed a herniation of the meninges into the thoracic cavity. The patient had widespread neurofibromatosis without associated kyphoscoliosis. Postoperative complications included paraplegia and cerebral edema, ultimately resulting in death. Through literature review, the study explores the pathogenesis, radiologic and clinical features, surgical indications, and potential complications of this rare entity.

Bu çalışma, Ankara Üniversitesi Tıp Fakültesi Göğüs ve Kalp Cerrahisi Kliniği’nde tanımlanan nadir bir intratorasik meningosel vakasını sunmaktadır. Hastada sol omuz ağrısı nedeniyle çekilen akciğer grafisinde arka üst mediastende kistik bir kitle gözlenmiş; başlangıçta nörojenik tümör düşünülmüş, ancak cerrahi sırasında meninkslerin toraksa fıtıklaştığı intratorasik meningosel olduğu anlaşılmıştır. Vakada yaygın nörofibromatozis bulunmasına rağmen kifoskolyoz saptanmamıştır. Cerrahi müdahale sonrası parapleji ve beyin ödemi gelişmiş, hasta ne yazık ki kaybedilmiştir. Literatür incelemesiyle birlikte hastalığın etiyolojisi, radyolojik özellikleri, cerrahi endikasyonları ve komplikasyonları detaylandırılarak gözden geçirilmiştir.
