Vulvar Leukoplakia: A Case Report and Surgical Approach to Precancerous Lesions

Abstract

Bu çalışma, vulva karsinomu ile ilişkili prekanseröz bir lezyon olan vulva lökoplakisinin klinik önemini vurgulamakta ve yaygın tutulumlu bir vakanın cerrahi yönetimini sunmaktadır. Olgu, 60 yaşındaki bir kadında 2 yıldır süren kaşıntı ve yeni başlayan kanama şikayetleriyle değerlendirilmiştir. Geniş alana yayılmış, keratozik ve granüler karakterdeki plaklar üzerine yapılan biyopsiyle vulvar lökoplaki tanısı doğrulanmış; histopatolojik incelemelerde malignite saptanmamış ancak yaygın ve semptomatik tutulum nedeniyle radikal tedavi (vulvektomi) uygulanmıştır. Yazarlar, lökoplakinin histolojik varyasyonlarına göre tedavi stratejilerinin farklılaşabileceğini; özellikle yaygın, şiddetli semptom veren ve malignite şüphesi taşıyan durumlarda radikal cerrahinin vulva kanseri profilaksisi açısından anlamlı olduğunu vurgulamaktadır.

Vulva Lökoplakisi: Bir Vaka Sunumu ve Prekanseröz Lezyonların Cerrahi Yaklaşımı

Abstract

This study emphasizes the clinical relevance of vulvar leukoplakia, a precancerous lesion often associated with vulvar carcinoma, and presents the surgical management of a widespread case. The patient—a 60-year-old woman—presented with chronic pruritus and recent bleeding. Examination revealed extensive keratotic and granular plaques, and biopsy confirmed vulvar leukoplakia without evidence of malignancy. Given the symptomatic and diffuse nature of the lesion, radical treatment (vulvectomy) was performed. The authors discuss treatment strategies based on histologic classification of leukoplakia, suggesting that radical surgery may play a vital role in cancer prophylaxis, particularly in extensive, symptomatic, or suspicious cases.
