Investigation of Liver Dysfunction in Passive Congestive Heart Failure with Hippuric Acid Test

Abstract

1. Hippuric acid values found with the original Ovick hippuric acid test in 20 normal individuals ranged from 1 to 1.4 g.
2. In 10 patients with various compensated valve lesions, the urinary hippuric acid excretion ranged from 1.1 to 1.4 g.

This value was 0.97 g in one patient, who was a borderline case between compensation and decompensation. Based on this result, slightly low hippuric acid test values can be taken as an early symptom in determining borderline points.
3. In 16 of 40 cases of passive congestive heart failure (4, 40), hippuric acid values were between 0.20 and 0.54 g before treatment. (In most of these patients, who did not benefit much from treatment, the liver did not shrink, and a slight decrease in hippuric acid values was observed after treatment compared to previous values), 15 (97, 37) had 0.54-0.75 g. (after treatment, the liver shrank, and hippuric acid values increased after treatment), and 9 (9, 23) had 0.75-0.89 g. (the liver shrank to a normal extent, and hippuric acid values increased to normal values after treatment).

4. The duration of the insufficiency, the frequency or infrequency of relapses, and therefore the degree of response to treatment, and finally, age, explain the change in hippuric acid values before and after treatment. 5. Generally, a decrease in the hippuric acid test value was observed in all patients with passive congestive heart failure.

Pasif Konjestif Kalb Yetmezliğinde Karaciğer Fonksiyon Bozukluğunun Hüpürik Asit Testiyle Araştırılması

Abstract

1. 20 normal kişide orijinal Ovick hipürik asit testi ile bulduğumuz
hipürik asit kıymetleri, 1 - 1,4 gr. arasındadır.
2. Çeşitli kompanse valvül leziyonu taşıyan 10 hastada, idrarla
çıkan hipürik asit, 1,1 - 1,4 gr. arasındadır.
Bir hastada bu kiymet 0,97 gr. idi ve bu hasta kompansasyon
ile dekompansasyon arasında bulunan bir sınır vakasıydı. Bu sonuca gö
re, hipürik asit testinin hafif düşük kıymetleri, sınır yakalarını tâyin işinde, bir erken semptom değerinde alınabilir.
3. Passif konjestif kalb yetmezliği gösteren 40 vakanın 16 sında
(4, 40), hipürik asit kıymetleri, tedaviden evvel, 0,20 - 0,54 gr. (tedaviden pek te istifade edemeyen bu hastaların çoğunluğunda karaciğer
küçülmediği gibi, hipürik asit kıymetlerinde de evvelki kıymetlere nazaran, tedaviden sonra, hafif bir düşme görüldü), 15 inde (97, 37) 0,54-
0,75 gr. (tedaviden sonra, karaciğer küçülmüş ve hipürik asit kıymetlerinde, tedaviden sonra, bir yükselme görülmüştür), 9 unda (9, 23) 0,75-
0,89 gr. (karaciğer normal denecek derecede ufalmış, hipürik asit kıymetleri tedaviden sonra, normal kıymetlere kadar yükselmiştir) bulunmuştur.
4. Yetmezliğin devam müddeti, nüküslerin oseyreklik veya sıklığı,
dolayısiyle tedaviye cevap verme derecesi, nihayet yaş, tedaviden evvel ve sonra hipürik asit kiymetlerindeki değişikliği izah eder.
5. Umumiyetle, passif konjestif kalb yetmezliği gösteren hastaların
hepsinde, hipürik asit testi kıymetinde bir azalma görülmüştür.
