Esansiyel Trombositopeni Tedavisinde Kortikosteroidlerle Elde Edilen Klinik Neticeler

Abstract

This study reports the clinical outcomes of corticosteroid therapy in 13 patients with idiopathic thrombocytopenic purpura treated at Ankara University Faculty of Medicine’s II. Internal Medicine Clinic between 1955 and 1960. Patients ranged in age from 13 to 50, with an average of 30 years. Prednisolone was administered in varying dosages (10–50 mg/day). Remission was achieved in 9 cases (69%), particularly in those with a shorter disease duration, suggesting a stronger therapeutic response in acute-stage patients. Corticosteroids effectively elevated platelet counts, reduced hemorrhagic symptoms and negative Lacét results were observed in most remitting cases. The remaining 4 cases showed no clinical improvement. Splenectomy was considered in refractory cases.

Clinical Results in the Treatment of Idiopathic Thrombocytopenic Purpura with Corticosteroids

Abstract

Bu çalışma, 1955–1960 yılları arasında Ankara Üniversitesi Tıp Fakültesi II. İç Hastalıkları Kliniği’nde kortikosteroid tedavisi uygulanan 13 esansiyel trombositopeni vakasının klinik sonuçlarını değerlendirmektedir. Yaşları 13 ile 50 arasında değişen hastalara prednisolon verilmiş, dozlar 10–50 mg arasında değişmiştir. 13 vakadan 9’unda (%69) klinik ve hematolojik remisyon sağlanmış olup, özellikle hastalık süresi kısa olan bireylerde tedaviye yanıt daha etkili olmuştur. Kortikosteroidler ile trombosit sayısı artmış, hemorajik diyatez düzelmiş ve Lacét delili çoğu remisyon vakasında menfileşmiştir. 4 vakada tedaviye yanıt alınamamıştır. Dirençli vakalarda splenektomi önerilmiştir.
