Renal İnfarktus Teşhisindeki Güçlükler (Bir Vak’a Münasebetiyle)

Abstract

Renal infarction is a rare yet diagnostically elusive condition due to its varied and often misleading symptom profile. This paper details a case involving a 61-year-old male who presented with left flank pain and signs mimicking renal tumor, but was ultimately diagnosed post-nephrectomy as having both recent and old infarcts in an arteriosclerotic kidney. The article explores historical and contemporary data including autopsy series, hemodynamic classifications (arterial, functional, venous), and anatomical pathology findings. Literature shows frequent misdiagnosis and emphasizes the role of nephrologists, urologists, and pathologists in collaborative evaluation. The case highlights discrepancies across diagnostic tests—IV pyelography, retrograde imaging, and perirenal insufflation—and argues for heightened suspicion of infarction in differential diagnoses. Conservative treatment is preferred, though nephrectomy remains viable in specific presentations.

Diagnostic Challenges of Renal Infarction (Based on a Case Report)

Abstract

Renal infarktus, nadir görülmesine rağmen semptomlarının çokluğu ve çeşitliliği nedeniyle teşhis aşamasında büyük zorluklar çıkaran bir klinik tablodur. Bu yazıda, sol böbrek ağrısı ve tümör şüphesiyle cerrahi müdahale uygulanan 61 yaşındaki erkek hastanın nefrektomi sonrası yapılan patolojik incelemesinde arteriosklerotik zeminde hem eski hem de yeni infarktus odaklarının tespit edilmesi sunulmuştur. Literatür verileri eşliğinde renal infarktus’un arteryel, fonksiyonel ve venöz mekanizmaları ile patolojisi ele alınmış; IV pyelografi, retrograd pyelografi ve perirenal ensüflasyon gibi tanı araçları arasındaki uyuşmazlık değerlendirilmiştir. Bu vak’a benzeri tablolarda renal tümör gibi görünüm veren infarktus olasılığının da mutlaka göz önünde bulundurulması gerektiği vurgulanmıştır. Tedavi açısından, mümkün olan durumlarda böbreğin korunması tercih edilmekte olup, bazı özel tablolar için nefrektomi önerilmektedir.
