Pheochromocytom (Bir Vak’a Münasebetiyle)
Abstract
This article presents a detailed case study of a 63-year-old woman diagnosed with pheochromocytoma—a chromaffin tumor originating from the adrenal medulla. Clinical symptoms included paroxysmal hypertension (up to 250 mmHg), tremors, palpitations, and glycosuria, with fluctuating epinephrine and norepinephrine secretion influencing blood pressure and metabolic functions. Diagnosis was supported by radiological findings, including perirenal insufflation and IV pyelography, which revealed a mass the size of a kidney above the right adrenal. Regitine testing confirmed the hypertension’s sensitivity to alpha blockade. Surgical excision was performed under tightly monitored anesthesia using intravenous agents and endotracheal oxygen—ether was avoided due to its sympathomimetic properties. Despite hemodynamic stabilization post-surgery, the patient died 24 hours later from shock due to diffuse hepatic necrosis. The case emphasizes the complexity of managing pheochromocytoma, particularly regarding intraoperative hypertension and anesthetic risk.
