Mide Tüberkülozu (Bir Vak’a Dolayısıyla Literatürün Gözden Geçirilmesi)
Abstract
This case report presents a rare form of gastric tuberculosis confirmed by surgery and histology. A 36-year-old male patient from Sivas was hospitalized with chronic postprandial epigastric pain, nausea, and vomiting. Past medical history included tuberculous adenitis and pleurisy. Physical examination showed epigastric tenderness and cervical lymph node scarring. Imaging revealed multiple gastric wall defects suggestive of malignancy, prompting gastroscopy and eventual laparotomy. Gastroscopy showed hypertrophic gastric folds, and surgery uncovered inflammatory masses and thickened gastric tissue. Histological analysis confirmed epithelioid histiocytes, Langhans giant cells, and caseating necrosis typical of tuberculosis. The literature review highlights diagnostic challenges due to symptom overlap with peptic ulcers and gastric carcinoma, outlines pathogenesis pathways including lymphatic spread, and notes complications such as pyloric obstruction and hemorrhage. The authors emphasize the importance of considering gastric TB in patients with prior tuberculosis history and nonspecific gastric symptoms.
