Splenectomy Indications

Abstract

In recent years, advances in the understanding of splenic physiology and hypersplenism have necessitated a reevaluation of the indications for splenectomy. This paper discusses the role of splenectomy in various clinical scenarios including traumatic and spontaneous splenic rupture, congenital and acquired hemolytic jaundice, idiopathic thrombocytopenic purpura, certain cases of aplastic anemia, and syndromes associated with hypersplenism. Additionally, the indications for surgery are examined in rare conditions such as congenital ovalocytosis, primary splenic neutropenia, Felty's syndrome, and collagen vascular diseases. The study emphasizes the importance of a multidisciplinary assessment—clinical, laboratory, and pathological—in selecting the appropriate patients for splenectomy.

Abstract

Son yıllarda dalak fizyolojisi ve hipersplenizme dair yeni bilgilerin ortaya çıkması, splenektomi endikasyonlarının yeniden değerlendirilmesini gerekli kılmıştır. Bu çalışmada, travmatik ve spontan dalak rüptürleri, konjenital ve kazanılmış hemolitik ikterler, idiopatik trombositopenik purpura, bazı aplastik anemiler ve hipersplenizmle ilişkili sendromlar gibi durumlarda splenektominin yeri ele alınmaktadır. Ayrıca konjenital ovalositoz, primer splenik nötropeni, Felty sendromu ve kollagen hastalıklarında cerrahi girişimlerin yeri vurgulanmaktadır. Bu endikasyonlar ışığında, doğru hasta seçiminde klinik, laboratuvar ve patolojik değerlendirmelerin bütüncül yaklaşımı önem arz etmektedir.
