Ankara'da Rastladığımız İki Trichostrongylose Vak'ası ve Bu Husustaki Tedavi Deneyleri

Abstract

This case report presents two rare instances of human Trichostrongylosis observed in Ankara and discusses the diagnostic methods and treatment attempts. Trichostrongylus spp. are nematode parasites typically affecting ruminants; human infestation is rare and acquired through ingestion of larvae-contaminated vegetables. Of 2348 stool examinations conducted at the Parasitology Institute, only two cases (%0.085) were identified—marking one of the earliest reports of the parasite in Türkiye since its initial discovery in 1955. Both patients belonged to a farming family in the village of İybeler (Kızılcahamam), closely exposed to livestock (sheep, goats, cattle) proven to carry the same parasites. Eggs of Trichostrongylus spp. were identified via native and concentration methods. Despite administering multiple drugs—piperazine hexahydrate, tetrachlorethylene, carbon tetrachloride, and piperazine adipate—no therapeutic success was achieved. The study underscores the difficulty in treating human Trichostrongylosis and the need for safer, more effective antiparasitic agents.

Two Cases of Trichostrongylosis Encountered in Ankara and Therapeutic Trials

Abstract

Bu çalışma, Ankara’da tespit edilen iki insan Trichostrongylose vak'ası ile ilgili klinik gözlemleri ve tedavi girişimlerini ele almaktadır. Trichostrongylus spp., genellikle geviş getiren hayvanların ince bağırsaklarında yaşar; insanlar için oldukça nadirdir. Enfeksiyon, larva taşıyan yeşilliklerin yenmesiyle oluşur. Parazit yumurtaları, nativ ve konsantrasyon yöntemleriyle gaitada tanımlanmıştır. Enstitüde yapılan 2348 dışkı muayenesinden sadece ikisinde parazite rastlanmıştır (%0,085). Vak'alardan biri 18 yaşındaki çiftçi bir kadın, diğeri annesidir; her ikisi de hayvanlarla yakın temasta bulunan bir kırsal aileye mensuptur. Kullanılan ilaçlar—piperazine hexahydrate, tetrachlorethylene, karbon tetraklorür ve piperazine adipate—tedavi açısından sonuç vermemiştir. Çalışma, insan Trichostrongylose enfeksiyonlarının tedavi zorluklarını ve mevcut ilaçların yetersizliğini vurgulamakta; daha etkili ve güvenli antiparaziter ajanlara ihtiyaç olduğunu belirtmektedir.
