Mide Kanserleri ve Abdomino-Torakal İki Safhalı Total Gastrektomi Tekniği

This article describes a modified two-stage abdomino-thoracic total gastrectomy technique for radical surgical treatment of gastric cancer. Reviewing literature and outcomes from 85 cases treated in the clinic, the author emphasizes the need for early diagnosis to improve operability and long-term survival, contrasting findings with prior international data: only 19% of cases were operable, while 13% received no intervention, and 18% underwent solely laparotomy. The proposed method separates surgery into two distinct stages: Abdominal Stage: Gastric tumor assessment via median or transverse incision; dissection of omenta and duodenum; resection of the stomach and spleen; and preparation of a jejunal loop with Braun anastomosis. Thoracic Stage (second or same day): Left 9th intercostal thoracotomy; mobilization of the esophagus and jejunum through diaphragm opening; and termino-lateral esophago-jejunostomy performed intra-thoracically. The technique avoids simultaneous pleural and peritoneal cavity exposure, allowing safer radical surgery. Two illustrated cases detail surgical steps, including complication management. The study concludes that two-stage gastrectomy offers significant advantages in operability, infection control, and adaptability to patient condition.

Özet: Bu yazı, mide kanserinin radikal cerrahi tedavisi için geliştirilen iki safhalı abdomino-torakal total gastrektomi tekniğini tanıtmaktadır. Literatür ve kliniğimizdeki 85 vak’a analiz edilerek, erken teşhis ile operabilitenin ve yaşam süresinin artırılabileceği vurgulanmıştır. Verilere göre vak’aların %19’u operabl, %13’üne müdahale edilmemiş, %18’ine yalnız laparatomi uygulanmıştır. Teknik iki aşamadan oluşmaktadır: Abdominal Safha: Göbek üstü ensizyon ile mide ve tümör durumu kontrol edilir; omentumlar ve duodenum disseke edilir; mide ve dalak çıkarılır; jejunum halkası hazırlanarak Braun anastomozu yapılır. Torakal Safha (aynı gün veya sonraki gün): 9. sol interkostal mesafeden göğse girilerek diyafragmadan jejunum ve özofagus göğüs içine alınır ve termino-lateral özofago-jejunostomi gerçekleştirilir. Bu yöntem, plevra ve periton boşluğunun aynı anda açılmasını engelleyerek enfeksiyon ve şok riskini azaltır. Yazıda iki detaylı vak’a ile cerrahi basamaklar ve komplikasyon yönetimi sunulmuştur. Sonuç olarak, bu teknik operabiliteyi artırmakta ve hasta durumuna göre uyarlanabilirliği ile önemli cerrahi avantajlar sağlamaktadır.
