Results from Our Shunt Surgery Ammoniaemia Issues in Shunt Surgery

Abstract

This study presents the outcomes of shunt surgeries (porto-caval and splenorenal) performed at the 2nd Surgical Clinic of Ankara University Faculty of Medicine and evaluates the risk of ammonia-induced hepatic coma in the surgical treatment of esophageal varices. The etiology and pathophysiology of portal hypertension, along with the mechanisms behind variceal formation, are thoroughly discussed. Various types of shunt procedures and their surgical techniques are illustrated through case studies. Four patient cases are reviewed, with one death attributed to hepatic coma. Additionally, the neurotoxic effects of ammonia accumulation and its treatment strategies are examined. The study emphasizes the importance of assessing portal circulation and hepatic function in surgical planning for bleeding esophageal varices.

Abstract

Bu çalışma, Ankara Tıp Fakültesi II. Cerrahi Kliniği'nde gerçekleştirilen shunt (porto-kaval ve splenorenal) ameliyatlarının sonuçlarını ve özofagus varislerinin cerrahi tedavisinde amonyakemiye bağlı hepatik koma riskini incelemektedir. Portal hipertansiyonun nedenleri, patofizyolojisi, özofagus varislerinin oluş mekanizmaları detaylı biçimde açıklanmıştır. Shunt ameliyatlarının endikasyonları, çeşitleri ve cerrahi teknikleri vaka örnekleriyle sunulmuştur. Toplam dört hasta vakası üzerinden uygulanan cerrahiler değerlendirilmiş, bir vakada hepatik koma nedeniyle ölüm bildirilmiştir. Ayrıca, amonyak birikiminin santral sinir sistemi üzerindeki toksik etkileri ve tedavi yöntemleri irdelenmiştir. Bu çalışma, cerrahi planlama sürecinde portal dolaşım ve hepatik fonksiyonların değerlendirilmesinin önemini vurgulamaktadır.
