The Values of Various Treatments in Liver Cirrhosis and Other Hepatitis C, Results from Hepatic Periarterial Neurectomy

Abstract

This article evaluates various surgical treatments applied to liver cirrhosis and other hepatic diseases at Ankara University’s 2nd Surgical Clinic. The focus is on the outcomes of hepatic periarterial neurectomy in patients with early and advanced cirrhosis. Among six reported cases, two patients demonstrated significant postoperative improvement, including regression of ascites, while two advanced cases resulted in hepatic coma and death. The study also discusses the role of other surgical interventions such as splenectomy, hepatic artery ligation, and portocaval shunts. The authors emphasize the importance of early diagnosis through liver biopsy and suggest that early application of hepatic periarterial neurectomy may halt or delay the progression of cirrhosis. Findings are interpreted in the context of related literature.

Karaci̇ğero Si̇rozu İle Di̇ğer Hepati̇tlerde Çeşi̇tli̇ Tedavi̇leri̇n Değerleri̇, Hepati̇k Peri̇arteri̇el Nörektomi̇den Aldiğimiz Neti̇celer

Abstract

Bu çalışma, Ankara Tıp Fakültesi II. Cerrahi Kliniği’nde gözlemlenen karaciğer sirozu ve diğer hepatit olgularında uygulanan çeşitli cerrahi yöntemlerin etkilerini değerlendirmektedir. Özellikle hepatik periarteriel nörektominin sirozun seyrine etkisi sorgulanmış; altı olgu üzerinden ameliyat öncesi ve sonrası bulgular sunulmuştur. Erken dönem vakalarda nörektominin olumlu sonuçlar verdiği, asitin gerilediği ve hastaların genel durumunda belirgin iyileşme gözlendiği belirtilmiştir. Ancak ileri evre sirozlu iki vakada postoperatif karaciğer koması ve ölüm gelişmiştir. Makalede, ayrıca diğer cerrahi girişimlerin (splenektomi, arteria hepatika ligasyonu, shunt uygulamaları) etkinliği de literatür ışığında tartışılmıştır. Yazarlar, sirozun preklinik evrede tanınması ve biyopsi ile erken teşhis konulan olgularda hepatik periarteriel nörektominin uygulanmasının potansiyel faydalarını vurgulamaktadır.
