Abstract

End-results base on the history, clinical examination, and roentgenologic study carried out from 1 to 6 years after operation in 18 patients with Mold arthroplasties were excellent and good in 79%, fair in 16% and poor in 5%. There was no operative mortality. There were seven rheumatoid arthritis and Marie - Strüm- pell arthritis, two degenerative osteoarthritis after subluxation of the hip and nine degenerative arthritis in our 18 cases. From this study, it is our believe that, with careful selection of patients, skilfull surgical technique and effectual postoperative care and rehabilitation vitallium Cup Arthroplasty of the hip has a definite place in the treatment of disabilities of the hip joint.

Kalça Artroplastisinde CUP (KAP) Artroplastisinin Yeri

Abstract

1961-1966 Seneleri arasında Ankara Tıp Fakültesi, Gülhane Askeri Tıp Akademisi ve özel serimizde olmak üzere 18 vakada yapılan cup artroplastisi dolayısıyla endikasyonlar ve rehabilitasyon konusu tartışıldı. İlk cup ameliyatı 1961 de Gülhane As. Tıp Akademisinde yapılmıştır. Sonuç olarak Smith Pettersen cup'ı ile artroplasti özellikle ankilozan tipteki artritlerde ve baş ve boyunda kısmen harabiyet yapan kalça osteoartritlerinde seçkin bir ameliyattır. Bunlarda başarı oranı muhtelif otörlere göre % 60-82 arasındadır. Bunlarda da revizyon ameliyatı gerekirse de hareketsiz ve ağrılı olarak ızdırap çeken bir kimseyi bir süre için dahi olsa rahat bir duruma sokmak bu ameliyat için yeterli bir endikasyondur. Artroplasti kalçanın diğer patolojik şartlarında kullanılırsa da % 10-35 oranında revizyon gerekir. Ve başarısızlık oranı otörlere göre % 30-50 arasındadır.
Biz 7 si romatoid ve Maria Strumpell tipi 9'u dejeneratif ve 2 si subluksasyon zemininde gelişen 18 kalça orteortritinde Smith Petersen cup'ı kullandık. Bunlardan birinde revizyon (%5) gerekti, 3 ünde orta şiddette ağrı veya ameliyattan hoşnutsuzluk (% 16) ve 14 ünde ameliyat sonucu iyi ve memnuniyet verici (% 79) olmuştur.
Özellikle Bursa'daki Milli Savunma Bakanlığı Rehabilitasyon Hastahanesinde rehabilitasyon imkânı sağlayan hastalarımızda daha kısa zamanda daha yüz güldürücü sonuç alınmıştır.
Sonuç olarak tekrar etmek gerekirse ameliyatın başarısı vakaların iyi seçilmesine usta bir cerrahi tekniğe, iyi bir rehabilitasyon ve hasta hekim iş birliğine bağlıdır.
