Gallbladder Perforations (Due to 4 cases)

Abstract

Gallbladder perforation into the free peritoneal space is a fairly common condition. These perforations occur most frequently in patients over 50 years of age with a history of cholelithiasis. However, perforation can also occur in patients with no previous symptoms. We have two such cases (Observations 1 and 2). Prodromal symptoms are often absent. Diagnosis is often not made before surgery. Increased intravesical pressure, as well as intravesical pancreatic juice regurgitation due to sphincter of Oddi dyskinesia, play a significant role in perforation. We have two such cases (Observations 3 and 4). The prognosis is quite poor, as the majority of cases are elderly, compromised, and present with generalized peritonitis. Treatment is immediate cholecystectomy and extensive peritoneal drainage. In %16-18 cases, choledocus stones may also be present, so a second surgery may be necessary in some cases.

Abstract

Safra kesesinin serbest periton boşluğuna delinmesi oldukça sık görülen bir haldir. Bu delinmeler daha çok 50 yaşını geçkin ve mazilerinde cholelithiasis hikâyesi bulunan hastalarda meydana gelir. Bununla beraber delinmenin daha önce hiçbir şikâyeti bulunmayan hastalarda da olması mümkündür. Bu şekilde iki vak'amız vardır (Müşahede 1-2). Prodrom belirtileri ekseriya yoktur. Ameliyattan önce teşhis ekseriya konulamaz. Delinme oluşunda kese içi basıncının artması kadar oddi sfenkteri
diskinezisine bağlı kese içi pankreas usaresi regurjitasyonlarının da rolü önemlidir. Bu şekilde iki vak'amız olmuştur (Müşahede 3 ve 4). Prognosis oldukça karanlıktır. Çünkü vak'aların en büyük kısmını yaşlı, direncini kaybetmiş ve genel peritonit entoksikasyonu ile gelen hastalar teşkil eder. Tedavi, acilen yapılacak koleksistektomi ve peritonun geniş bir şeklide drenajıdır. Vak'aların %16-18 inde aynı zamanda choledocus taşıda olabileceği için bazı hallerde ikinci bir ameliyata lüzum olabilir.
