Apexcardiogram (APG) in Mitral Valve Diseases

Abstract

This study investigates the diagnostic value of the apexcardiogram (AKG) in differentiating mitral valve lesions, specifically mitral stenosis and mitral insufficiency, through an analysis of diastolic phase characteristics. Conducted at the Cardiology Section of the 1st Internal Medicine Clinic, Ankara University Faculty of Medicine, the research includes 40 patients diagnosed with mitral valvular disease. Patients were grouped based on whether stenosis or insufficiency was predominant. The study focused on diastolic filling waves and their angular measurements (R*, S*, T*) and ratios (T*/R*, T*/S*, RFW/T) on AKG. Findings indicated that patients with dominant stenosis displayed flat or absent rapid filling waves and more prominent slow filling phases, whereas those with dominant insufficiency exhibited high and steep rapid filling waves. Statistical analysis of AKG parameters demonstrated their usefulness in distinguishing between the two lesion types. The study concludes that AKG provides clinically relevant and non-invasive data, assisting in the determination of appropriate surgical interventions for mitral valve disease.

Mi̇tral Valvül Hastaliklarında Apekskardi̇yogram (Akg)

Abstract

Bu çalışma, mitral valvül hastalıklarında (mitral darlığı ve mitral yetmezliği) apekskardiyogramın (AKG) diyastolik fazlar üzerindeki ayırt edici etkilerini ortaya koymak amacıyla gerçekleştirilmiştir. Ankara Üniversitesi Tıp Fakültesi I. İç Hastalıkları Kliniği Kardiyoloji Seksiyonu’nda yapılan bu araştırmaya, mitral valvül lezyonu tanısı almış 40 hasta dahil edilmiştir. Hastalar, mitral darlığının veya yetmezliğinin baskın olduğu klinik bulgulara göre dört gruba ayrılmıştır. AKG üzerinde diyastolik doluş fazlarına ait açılar (R*, S*, T*) ve bu fazlara ilişkin oranlar (T*/R*, T*/S*, RFW/T) hesaplanmıştır. Bulgulara göre, mitral darlığı baskın olan vakalarda yavaş doluş dalgası baskın ve süratli doluş dalgası silik veya yok iken, mitral yetmezliği baskın vakalarda yüksek ve dik süratli doluş dalgaları kaydedilmiştir. AKG parametreleriyle yapılan istatistiksel karşılaştırmalar, bu iki lezyon tipinin ayrımında AKG’nin anlamlı veriler sunduğunu göstermiştir. Sonuç olarak, mitral valvül hastalıklarında AKG’nin, operasyon şekline karar verme sürecinde faydalı bir non-invaziv tanı aracı olduğu vurgulanmaktadır.
