Malignant Teratoma and Seminoma in Testicular Fusion Anomaly (Synorchism)

Abstract

This paper presents a rare case of testicular fusion anomaly (synorchism) observed at the Ankara University Medical Faculty Oncology Institute and Hospital, accompanied by two distinct testicular malignancies. A 35-year-old male patient, with a congenital absence of the right testis, developed a progressively enlarging mass in the left testicle and abdominal and supraclavicular regions. Clinical and pathological evaluations revealed a seminoma in the left testis and a malignant teratoma in the right-sided abdominal mass. Histological analysis of the surgically resected tissues confirmed the presence of two separate tumors with different cellular origins. This unique case demonstrates the co-occurrence of a testicular fusion anomaly and two distinct germ cell tumors—an extremely rare presentation in urologic oncology. The case is discussed in the context of the existing literature, with emphasis on the diagnostic challenges, operative findings, histopathologic features, and treatment course.

Testi̇s Füzyon Anomali̇si̇nde (Synorchi̇sm) Mali̇gne Teratom Ve Semi̇nom

Abstract

Bu makalede, Ankara Üniversitesi Tıp Fakültesi Onkoloji Enstitüsü ve Hastanesi’nde gözlenen, oldukça nadir bir testis füzyon anomalisi (synorchism) ve bu anomalinin her iki testisinde farklı malign tümörler gelişmiş bir olgu sunulmaktadır. 35 yaşındaki erkek hastada doğuştan beri sağ testis mevcut değilken, sol testiste son bir yıl içinde ağrılı büyüme ve vücudun farklı bölgelerinde kitleler gelişmiştir. Klinik, radyolojik ve laboratuvar incelemeleri sonucunda; sol skrotal testiste seminom, sağ abdominal bölgede ise malign teratom geliştiği belirlenmiştir. Operasyonla çıkarılan testis dokularının histopatolojik incelemesi, iki farklı tümör yapısını doğrulamıştır. Bu durum, literatürde nadir bildirilen “synorchism” zemininde her iki testiste farklı histogenezli malignite gelişimiyle dikkat çekmektedir. Vaka, hem konjenital anomalinin seyrekliği hem de tümör tiplerinin farklılığı açısından literatüre katkı sunmakta; tanı, cerrahi yaklaşım, histolojik değerlendirme ve tedavi süreci ayrıntılı olarak tartışılmaktadır.
